Исследование посвящено определению диагностических возможностей контрастной стресс-эхокардиографии (КС-ЭхоКГ) с дипиридамолом у больных ишемической болезнью сердца (ИБС). Материал и методы. Обследованы 22 пациента с типичными клиническими проявлениями стенокардии, средний возраст 56,04±1,8 лет. В сравнительном аспекте рассматриваются результаты традиционной стресс-ЭхоКГ (С-ЭхоКГ) с дипиридамолом и КС-ЭхоКГ по отношению к данным коронароангиографии (КАГ). Результаты. Продемонстрировано, что оба метода ЭхоКГ визуализации высокоинформативны в плане топической диагностики коронарного атеросклероза – 91% по отношению к результатам КАГ. Выполнение КС-ЭхоКГ имеет ряд существенных преимуществ. Чувствительность, специфичность и диагностическая ценность КС-ЭхоКГ по сравнению с традиционной С-ЭхоКГ составляют 95% и 75%, 95% и 86%, 50% и 91%, соответственно. В отличие от стандартной С-ЭхоКГ с дипиридамолом при КС-ЭхоКГ важное значение имеет факт появления нарушений перфузии на малых дозах дипиридамола, что позволяет визуализировать угнетение коронарного кровообращения раньше, чем могут появиться зоны асинергии. Заключение. КС-ЭхоКГ с дипиридамолом обладает высокой диагностической ценностью в определении коронарной недостаточности с различной степенью поражения коронарного русла. Результаты КСЭхоКГ с дипиридамолом сопоставимы с результатами КАГ.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To assess diagnostic potential of contrast stress echocardiography (CS-EchoCG) with dipyridamole in coronary heart disease (CHD) patients. Material and methods. The study included 22 patients (mean age 56.04±1.8 years) with typical CHD clinics. The results of traditional S-EchoCG and CS-EchoCG with dipyridamole were compared with coronaroangiography (CAG) data. Results. Both methods of EchoCG visualization were highly informative for topic diagnostics of coronary atherosclerosis – 91%, comparing with CAG results. CS-EchoCG had a number of benefits: sensitivity, specificity, and prognostic value, comparing to traditional S-EchoCG, were 95% and 75%, 95% and 86%, 50% and 91%, respectively. In CS-EchoCG, perfusion abnormalities were registered even with low dipyridamole doses. This allows to diagnose coronary blood flow disturbance before asynergic zones appear. Conclusion. CS-EchoCG with dipyridamole was highly predictive in coronary insufficiency diagnostics.
