Определение оптимальной тактики антигипертензивной терапии (АГТ) у пациентов с гипертоническим кризом (ГК), осложненным геморрагическим мозговым инсультом (ГМИ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. 40 пациентов в возрасте 46–87 лет, госпитализированных с ГК, осложненным ГМИ.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты были разделены на 2 группы (гр.) по медиане систолического артериального давления (САД) к 20 мин от начала лечения: САД &gt;161 и САД &lt;161. Оказалось, что достоверно лучшую выживаемость имели пациенты с более выраженным снижением САД к 20 мин. Далее для определения оптимального уровня АД пациенты были разделены на 3 гр. по терцилям САД к 20 мин от начала. Оказалось, что достоверно лучшую выживаемость имели пациенты, достигшие к 20 мин САД 136–149 мм рт.ст. Величина САД к 220 мин на выживаемость достоверно не влияла. Далее пациенты были разделены на 2 гр. по скорости снижения САД. Выживаемость оказалась лучше в гр. пациентов с большей скоростью снижения САД (РGW=0,002). Более медленное снижение САД ассоциировалось с худшим прогнозом. Вторая часть исследования заключалась в оценке безопасности и эффективности препарата урапидила гидрохлорида (Эбрантил). Пациенты были рандомизированы на 2 гр.: лечения Эбрантилом и гр. стандартной терапии, принятой в отделении нейрореанимации. Оказалось, что Эбрантил обеспечивал быстрое и эффективное снижение АД к 20 мин от начала лечения, что обуславливало лучшую выживаемость.</p></sec><sec><title>Заключение</title><p>Заключение. Был определен оптимальный уровень АД для пациентов, госпитализированных с ГК, осложненным ГМИ, который ассоциировался с лучшей выживаемостью. Целевой уровень АД должен быть достигнут уже к 20 мин лечения, более медленная скорость снижения АД связана с худшим прогнозом. Эбрантил эффективный и безопасный антигипертензивный препарат.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To identify the optimal tactics of antihypertensive therapy (AHT) in patients with hypertensive crise (HC), complicated by haemorrhagic stroke (HS).</p></sec><sec><title>Material and methods</title><p>Material and methods. Forty patients aged 46–87 years, who were hospitalised with HC and HS as its complication.</p></sec><sec><title>Results</title><p>Results. All patients were divided into two groups, according to the median levels of systolic blood pressure (SBP) at 20 minutes after the start of the treatment: SBP &gt;161 mm Hg vs. SBP &lt;161 mm Hg. A significantly better survival was observed in patients with a higher degree of the 20-minute SBP eduction. In addition, patients were divided into tertiles of the 20-minute SBP levels. Participants with SBP 136–149 mm Hg demonstrated a significantly better survival. The degree of SBP reduction at 220 minutes was not significantly associated with survival. Furthermore, the patients were divided into two groups by the rate of SBP reduction. Faster rates were associated with a better survival (PGW=0,002). The second part of the study was focused on the assessment of effectiveness and safety of urapidil hydrochloride (Ebrantil). The patients were randomised into two groups: one group was administered Ebrantil, while another received the standard neuroresuscitation unit treatment. Ebrantil therapy demonstrated a fast and effective SBP reduction at 20 minutes, which was also associated with a better survival.</p></sec><sec><title>Conclusion</title><p>Conclusion. We identified optimal, associated with a better survival, BP levels (136–149 mm Hg) for patients with HC complicated by HS. Target BP levels should be achieved within 20 minutes, as a slower BP reduction is linked to a worse prognosis.
