Изучить прогностическое значение уровней растворимой формы лиганда CD40 (sCD40L) в плазме крови больных с различными формами ишемической болезни сердца (ИБС).Материал и методы. Обследованы 65 больных ИБС, среди них: 26 больных стабильной стенокардией  напряжения (ССН) I-III ФК, 23 – нестабильной стенокардией (НС) II-III В стадий и 15 – больных инфарктом миокарда (ИМ). Определяли уровни sCD40L, высокочувствительного C-реактивного белка, интерлейкина-6 и фибриногена. Период наблюдения составил 24 месяца. Конечными точками исследования рассматривали смерть от любых причин, развитие нефатального ИМ, ишемического инсульта, операцию по реваскуляризации миокарда и госпитализацию по поводу прогрессирования ССН или развития НС.Результаты. Уровни sCD40L практически не различались в подгруппах больных ССН, НС и ИМ. Пациенты с сахарным диабетом 2 типа (СД-2) имели достоверно более высокие концентрации маркера по сравнению с остальными больными ИБС. Сравнение выживаемости без нежелательных сердечно-сосудистых событий (ССС) выявило, что в группе больных ИБС с низкими уровнями sCD40L выживаемость достоверно лучше, чем у пациентов с высокими концентрациями маркера. Различие выживаемости между группами сохранялось и при исключении из анализа больных СД-2.Заключение. Исходно повышенные уровни sCD40L являются независимыми предикторами развития нежелательных ССС в течение 2 лет у больных ИБС.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To study prognostic value of plasma soluble CD40 ligand (sCD40L) level in patients with various coronary heart disease (CHD) forms.Material and methods. In total, 65 CHD patients were examined, including 26 with stable effort angina  (SEA), functional class (FC) I6III; 23 – with unstable angina (UA), Stage II6IIIB; 15 – with myocardial infarction (MI). Levels of sCD40L, highly sensitive C-reactive protein (hsCRP), interleukin-6 (IL-6), and fibrinogen were measured. Follow-up period lasted for 24 months. End-points included death from any cause, non6fatal MI, ischemic stroke, myocardial revascularization, hospitalization due to SEA progression or UA development.Results. In SEA, UA and MI patients, sCD40L levels were similar. Type 2 diabetes mellitus (DM-2) patients had significantly higher sCD40L levels, comparing to other CHD participants. Cardiovascular event (CVE)-free survival analysis demonstrated that in CHD patients with low sCD40L levels, survival was significantly better than in participants with its higher levels, even after DM-2 patients' exclusion.Conclusion.
