Оценить локальную сократимость (ЛС) миокарда левого и правого желудочков (ЛЖ, ПЖ) и ее изменение в условиях острой лекарственной пробы (ОЛП) с нитроглицерином (НГ) у больных с рецидивирующей формой фибрилляции предсердий (ФП) в период сохранения синусового ритма. Материал и методы. В исследование включены 120 больных в возрасте 39-82 лет (средний возраст 61,3±8,9), среди которых 20 пациентов (IА группа) с рецидивирующей формой ФП на фоне артериальной гипертензии, 50 больных (IБ группа) ишемической болезнью сердца и рецидивирующей формой ФП, 50 человек (II группа) – группа сравнения. Для оценки инотропной функции миокарда всем больным проводилась равновесная радионуклидная вентрикулография. Изучение состояния ЛС ЛЖ и ПЖ проводилось на основе унифицированной 16 сегментарной модели желудочков. После регистрации исходных показателей гемодинамики у больных повторно оценивались показатели ЛС в условиях ОЛП с НГ – через 30 минут после сублингвального приема 0,5-1 мг НГ. Результаты. При изучении ЛС миокарда у больных IА и IБ групп выявлены зоны гипо- и акинеза. В условиях ОЛП с НГ у больных IА и IБ групп улучшилась ЛС в виде достоверного уменьшения количества акинетичных сегментов и увеличения зон нормокинеза ЛЖ и ПЖ. У пациентов II группы при проведении ОЛП с НГ ЛС увеличилась за счет гипокинетичных сегментов. Заключение. Изменения ЛС у больных с рецидивирующей формой ФП в период сохранения синусового ритма представлены зонами гипо- и акинеза. При проведении ОЛП с НГ у больных было отмечено достоверное улучшение ЛС за счет уменьшения числа акинетичных сегментов, в группе сравнения наблюдалось достоверное уменьшение количества гипокинетичных сегментов.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To evaluate local contractility of left and right ventricular (LV, RV) myocardium, as well as its dynamics in acute drug test (AMT) with nitroglycerin (NG) in patients with recurrent atrial fibrillation (AF), during sinus rhythm maintenance period. Material and methods. The study included 120 patients aged 39-82 years (mean age 61.3±8.9 years): 20 patients with recurrent AF and arterial hypertension (AH) (Group IA); 50 patients with coronary heart disease (CHD) and recurrent AF (Group IB); 50 individuals from control group (Group II). To assess inotropic myocardial function, balanced radionuclide ventriculography was performed in all participants. Local LV and RV contractility was studied in unified 16-segment ventricular model. After registering baseline hemodynamic parameters, local contractility was assessed again in ADT with NG – 30 minutes after sublingual NG administration (0.5-1.0 mg). Results. In Groups IA and IB, local hypo- and akinesis were observed. In ADT with NG, patients from Groups IA and IB demonstrated improved local contractility: akinetic segment number significantly reduced, normokinetic LV and RV zone number substantially increased. In Group II, during ADT with NG, local contractility increased due to hypokinetic segments. Conclusion. Local contractility disturbances in patients with recurrent AF during sinus rhythm maintenance were manifested as hypo- and akinetic zones. In ADT with NG, patients demonstrated significant improvement of local contractility, due to decreased number of akinetic segments.
