Сравнить эффекты Заместительной гормональной (ЗГТ) и антигипертензивной терапии (АГТ) на метаболический профиль, инсулинорезистентность (ИР), состояние центральной гемодинамики у пациенток с метаболическим синдромом (МС) в постменопаузе.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Обследованы 46 пациенток с МС в постменопаузе. Пациентки I группы (п=30) в течение 6 мес. получали эстрадиол 1 мг/сут. + дроспиренон 2 мг/сут. (препарат Анжелик). Пациенткам II группы (п=16) назначали различные антигипертензивные препараты. До включения в исследование и через 6 мес. лечения определяли липидный профиль, гликемию и инсулинемию, индекс ИР HOMA-IR, композиционный состав тела, индекс Купермана в модификации Вихляевой Е.М, выполняли суточное мониторирование артериального давления, эхокардиографию.</p></sec><sec><title>Результаты</title><p>Результаты. У пациенток I группы снизился индекс массы тела (ИМТ) с 30,9 кг/м2 до 30,2 кг/м2 (р=0,068); во II группе наблюдали увеличение ИМТ с 30,6 кг/м2до 31,3 кг/м2 (р=0,003). В I группе показатель процентного содержания висцеральной жировой ткани соответствовал 40,6 % исходно и 42,2 % через 6 мес. терапии (р=0,002); во II группе до лечения количество висцеральной жировой ткани составило 41,0 %, после лечения — 42,2 % (р=0,018). Препарат Анжелик продемонстрировал хорошую антигипертензивную эффективность. Негативное влияние ЗГТ на метаболический профиль отсутствовало.</p></sec><sec><title>Заключение</title><p>Заключение. Анжелик эффективен и безопасен у пациенток с высоким риском сердечно-сосудистых осложнений. Отсутствовало негативное влияние препарата на метаболический профиль, ИР. За счет антиминералокортикоидного эффекта происходила коррекция параметров центральной гемодинамики и снижение АД.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. То compare the effects of hormone replacement therapy (HRT) and antihypertensive therapy (AHT) on metabolic profile, insulin resistance (IR) and central hemodynamics in postmenopausal women with metabolic syndrome (MS).</p></sec><sec><title>Material and methods</title><p>Material and methods. In total, 46 postmenopausal women with MS were examined. Group I (n=30) received estradiol (1 mg/day) and drospirenone (2 mg/day) for 6 months (Angeliq medication). Group II (n=16) received various antihypertensive medications. At baseline and after 6 months of the treatment, lipid profile, glycemia and insulinemia, IR index HOMA-IR, body composition, Kuperman index (modified by E..M. Vikhlyaeva) were assessed, 24-hour blood pressure monitoring (BPM) and echocardiography were performed.</p></sec><sec><title>Results</title><p>Results. In Group I, body mass index (BMI) reduced from 30,9 kg/m2 to 30,2 kg/m2 (p=0,068); in Group II, it increased from 30,6 kg/m2to 31,3 kg/m2 (p=0,003). Visceral fat tissue percentage was 40,6 %. at baseline and 42,2 % after 6 months in Group I, comparing to 41,0 % and 42,2 %, respectively, in Group II (p=0,018). Angeliq medication demonstrated good antihypertensive activity. In Group I, left ventricular myocardial mass index (LVMMI) significantly reduced from 116,1 g/m2 to 110,8 g/m2 (p&lt;0,0001); in Group II, it decreased from 116,4 g/m2 to 112,7 g/m2 only (p=0,062). HRT did not affect metabolic profile.</p></sec><sec><title>Conclusion</title><p>Conclusion. Angeliq medication was effective and safe in women with high cardiovascular risk, with no adverse effect on metabolic profile or IR.
