Для уточнения прогностической роли ЭД был выполнен сравнительный анализ работ по этому вопросу с использованием различных методов оценки эндотелиальной функции (ЭФ). Исследования, анализирующие ЭФ, продемонстрировали четкую взаимосвязь ЭД с различными клиническими исходами сердечно-сосудистых заболеваний (ССЗ). В настоящее время основным методом определения вероятности развития различных сердечно-сосудистых событий в ближайшие 5–10 лет у больных ССЗ и у лиц без клинической манифестации ССЗ, является оценка суммарного риска смерти. Можно предположить, что ЭД является независимым маркером атеросклеротического процесса даже при его субклиническом течении и может служить предвестником различных сердечно-сосудистых событий. Вероятно, эндотелий выступает в роли необходимого связующего звена между любым ФР и его отрицательным влиянием на сосудистую стенку. Оценка ЭФ может широко использоваться для динамического наблюдения за пациентами в качестве одного из показателей эффективности профилактических вмешательств, что предстоит подтвердить в ходе дальнейших научных исследований.</p></abstract><trans-abstract xml:lang="en"><p>Endothelial dysfunction (ED) amplifies the effects of vascular risk factors (RF), and is directly linked to pathogenetic mechanisms of atherosclerosis and its complications. To clarify ED prognostic role, a review of relevant studies was performed. ED interrelation with various clinical outcomes of cardiovascular disease (CVD) was demonstrated. At the moment, the main method of 5-10-year CVD risk assessment in CVD patients and asymptomatic individuals is evaluation of total mortality risk. ED could be an independent marker of atherosclerosis, including its subclinica course, and a predictor of various CVD events. Endothelium may link various RF and their damaging effect on vascular wall.
