Выяснить, как связаны показатели инсулинорезистентности (ИР) и липид-транспортной системы (ЛТС) у пациентов с нормогликемией натощак. Материал и методы. У 39 пациентов 20-70 лет с нормальной концентрацией глюкозы в крови натощак определены уровни липидов и аполипопротеинов (апо), а также показатели ИР: индекс инсулиночувствительности и индекс UP (ISI McAuley, HOMA-IR). Результаты. Пациенты с нормогликемией натощак и низким ISI McAuley имели атерогенную дислипопротеидемию: более высокие уровни триглицеридов, апо В, апо В/апо АI, меньший размер частиц липопротеинов низкой плотности; изменения показателей липопротеинов высокой плотности (ЛВП)-опосредованного транспорта холестерина (ХС): более низкий уровень свободного и эстерифицированного ХС ЛВП, фосфолипидов, апо А1, меньшую загруженность частиц ЛВП ХС и более высокое концентрационное отношение апо АII/апо AI. Заключение. У пациентов с нормальным уровнем глюкозы натощак выявление скрытых нарушений углеводного обмена, ассоциированных с изменениями ЛТС, которые могут способствовать развитию ишемической болезни сердца и сахарного диабета 2 типа, позволит начать коррекцию метаболических нарушений на ранних стадиях этих заболеваний.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To investigate the link between insulin resistance (IR) and lipid transport system in patients with fasting normoglycemia. Material and methods. In 39 patients aged 2070 years, with fasting normoglycemia, the levels of lipids, apolipoproteins (apo), and IR parameters (insulin sensitivity index; ISI McAuley, HOMA-IR) were measured. Results. In patients with fasting normoglycemia and low ISI McAuley, atherogenic dyslipoproteinemia was observed: increased triglycerides, apo B, apo B/apo AI levels, reduced low-density lipoprotein particle size; disturbed high-density lipoprotein (HDL)-mediated cholesterol (CH) transport: reduced levels of free and esterified HDL-CH, phospholipids, apo A1, decreased HDL-CH particle loading, increased apo AII/apo AI concentration ratio. Conclusion.
