Оценить в ходе длительного, проспективного наблюдения за подростками динамику, устойчивость и прогностическое значение показателей липидного спектра (ЛС) крови для обоснования подходов к ранней профилактике ишемической болезни сердца. Материал и методы. В исследование изначально вошли 347 мальчиков и 332 девочки. За 22-летний период наблюдения проведено 6 повторных обследований в возрасте: 13-14, 15-16, 19-20, 21-22, 26 и 34-35 лет с определением ЛС. Обследование включало: определение общего холестерина (ОХС), ХС липопротеинов высокой плотности (ЛВП) и триглицеридов (ТГ), а также измерение массы (МТ) и длины тела (ДТ), толщины кожных складок под лопаткой (КСЛ), на животе (КСЖ) и над трицепсом (КСТ), оценку полового созревания (ПС). Результаты. Значительная доля различий в показателях ЛС крови между лицами мужского и женского пола, особенно в пубертатном возрасте, оставалась необъясненной. Увеличение атерогенности ЛС крови и частоты дислипидемий (ДЛП) у мужчин в молодом взрослом возрасте было сопряжено с увеличением МТ и ее жировой компоненты. Умеренно повышенные и нормальные уровни ХС, ХС ЛВП и ХС липопротеинов низкой плотности (ЛНП) у мальчиков и девочек пубертатного возраста имели одинаковую устойчивость и не ассоциировались с повышенным риском атерогенных ДЛП в зрелом взрослом возрасте. ОХС у подростков в периоде ПС имел прогностическое значение в отношении его уровня в возрасте 34-35 лет только в сочетании с показателями физического развития. Избыточная МТ у мальчиков пубертатного возраста являлась независимым предиктором будущей гиперхолестеринемии в зрелом взрослом возрасте. Заключение. Особенности возрастной динамики показателей ЛС крови служат основанием для начала профилактического вмешательства в детском возрасте – в препубертатном или раннем пубертатном периодах не только в группах риска, но и на популяционном уровне.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To prospectively assess dynamics, stability, and prognostic value of blood lipid profile (LP) in adolescents, for identifying early coronary heart disease prevention approaches. Material and methods. This prospective study included 347 boys and 332 girls. During 22-year follow-up, six clinical examinations were performed (at the age of 13-14, 15-16, 19-20, 21-22, 26 and 34-35 years). Examination program included measuring levels of total cholesterol (TCH), high-density lipoprotein CH (HDL-CH), and triglycerides (TG), body weight (BW) and height (BH), subscapular, abdominal, and triceps skin fold thickness (SST, AST, and TST), pubescence assessment. Results. Gender differences in LP, especially in pubescence, could not be completely explained. Increased LP atherogenity and dyslipidemia (DLP) rates in young males were associated with increased BW and fat tissue percentage. Mildly increased and normal TCH, HDL-CH, low-density lipoprotein CH (LDL-CH) in pubescent boys and girls were equally stable and not associated with increased risk of atherogenic DLP in adult age. In pubescent adolescents, TCH predicted its level in 34-35 year-olds only in combination with physical development parameters. Overweight in pubescent boys was an independent predictor of future hypercholesterolemia in adult age.
