Современные терапевтические возможности — диуретики, вазодилататоры, инотропные средства эффективно устраняют симптомы ОСН, но не снижают риск смерти, который при этом неотложном состоянии остается очень высоким. Цель. Оценить эффективность серелаксина при ОСН вследствие кардиотоксического эффекта цитостатической терапии. Материал и методы. Серелаксин — рекомбинантный человече­ский релаксин-2, обладающий как гемодинамическими, так и плей- отропными органопротективными эффектами. Данные об эффек­тивности препарата в зависимости от причины ОСН в настоящее время ограничены. В одноцентровом, наблюдательном исследова­нии представлен опыт применения серелаксина при отеке легких, развившемся как осложнение полихимиотерапии (ПХТ) при онкоге- матологических заболеваниях.Результаты. Применение серелаксина в дополнение к стандартно­му лечению позволило эффективно купировать отек легких, стаби­лизировать гемодинамические показатели и газовый состав крови и завершить программу ПХТ в полном объеме. Заключение. Представленный первый клинический опыт показал эффективность серелаксина при ОСН вследствие кардиотоксиче- ского действия ПХТ у больных онкогематологическими заболевани­ями. Целесообразны дальнейшие клинические исследования сере­лаксина у данной когорты больных. </p></abstract><trans-abstract xml:lang="en"><p>Treatment of acute heart failure (ACF) remains a strenuous task. Modern treatment methods — diuretics, vasodilators, inotropic drugs — are effective in ACF symptoms reduction, but do not decrease risk of death, that is still very high in this urgent condition. This determines a necessity of novel treatment methods search.</p><sec><title>Aim</title><p>Aim. To assess efficacy of serelaxine in ACF caused by cardiotoxic effect of cytostatic therapy.</p></sec><sec><title>Material and methods</title><p>Material and methods. Serelaxine is a recombinant human relaxine-2, having hemodynamic and pleiotropic organoprotective effects. The data on the drug efficacy for one or other cause of ACF is non-sufficient. In single-center observational study there is an experience presented of serelaxine in pulmonary oedema, developed as complication of polychemotherapy (PCT) in oncohematological diseases.</p></sec><sec><title>Results</title><p>Results. The use of serelaxine in addition to the standard treatment led to successful cessation of pulmonary oedema, stabilization of hemodynamics and of blood gases, therefore to completion of full PCT program.</p></sec><sec><title>Conclusion</title><p>Conclusion. The first clinical experience presented has shown efficacy of serelaxine in ACF due to cardiotoxic action of PCT in oncohematological patients.
