Цель. Определить структуру нарушений ночного сна у пациентов с эссенциалъной артериальной гипертензией (ЭАГ) и возможное влияние этих нарушений на течение заболевания.

Материал и методы

Материал и методы. Обследованы 98 пациентов с ЭАГ I и II степеней. Все больные прошли стандартное клиническое обследование, суточное мониторирование артериального давления (АД), анкетирование на предмет выявления нарушений ночного сна, определение уровня цейтнотности.

Результаты

Результаты. У пациентов с ЭАГ наиболее выражены и чаще всего встречаются интрасомнические и пре-сомнические расстройства, такие как нарушения засыпания, частые ночные пробуждения, храп, неглубокой поверхностный сон; 50 % пациентов оценивают общее качество своего сна, как хорошее, 35,71 % - как "не очень", 10,20 % - как плохое, и только 4,08 % - как отличное. Среди пациентов с ЭАГ существует небольшая группа лиц, у которых наиболее выражены нарушения ночного сна, повышен уровень тревожности, значительно изменен суточный профиль АД (non-dippers, night pikers).

Заключение

Заключение. У пациентов с ЭАГ достоверно чаще встречаются сомнические нарушения. Эти нарушения прогрессируют по мере увеличения продолжительности заболевания и его тяжести. При этом на более ранних стадиях болезни превалируют интра- и пресомнические расстройства. В дальнейшем, по мере увеличения длительности и прогрессирования заболевания нарушается вся структура сна.

Aim

Aim. To investigate nighttime sleep disturbances in patients with essential arterial hypertension (EAH) and possible effects of these disturbances on EAH clinical course.

Material and methods

Material and methods. In total, 98 patients with Stage I-II EAH were examined. All participants underwent standard clinical examination, 24-hour blood pressure monitoring (BPM), questionnaire survey on nighttime sleep disturbances, and time deficit level measurement.

Results

Results. EAH patients were characterized by frequent intrasomnic and presomnic disturbances, such as difficulty falling asleep, frequent night awakening, snoring, and superficial sleep. Fifty per cent of the patients regarded general quality of their sleep as "good", 35,71 % - as "fair", 10,20 % - as "bad", and only 4,08 % - as "very good". A group of EAH patients was identified, characterised by severe nighttime sleep disorders, elevated anxiety level, and disturbed circadian BP profile (non-dippers, night-peakers).

Conclusion

Conclusion. EAH patients often suffered from somnologic disturbances, which progressed with longer duration and more severe clinical course of the disease. For early EAH stages, intra- and presomnic disturbances were typical, with further development of general sleep structure disorders.
