Изучить предикторы внезапной кардиальной смерти (ВКС) у больных, перенесших инфаркт миокарда (ИМ), разработать стратификацию риска и пути профилактики ВКС.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Обследованы 420 больных на 10-14 день ИМ, проспективное наблюдение за больными осуществлялось в сроки 1-4 года. Больным выполнялись общеклиническое обследование, эхокардиография, суточное мониторирование ЭКГ, определение поздних потенциалов желудочков (ППЖ), активная ортостатическая проба (АОП), оценка вариабельности сердечного ритма (ВСР) в покое и при вегетативных пробах, психологическое тестирование, выборочно - коронарография и эндокардиальное электрофизиологическое исследование</p></sec><sec><title>Результаты</title><p>Результаты. Наиболее значимыми факторами оказались 6 следующих признаков: наличие ППЖ, сниженная ВСР, фракция выброса левого желудочка &lt; 40 %, желудочковые нарушения ритма, повторный ИМ, гипотензия при АОП. Предсказательная точность и положительная прогностическая ценность данной модели были весьма высокими, позволяя прогнозировать риск ВКС. Предложена стратификация риска ВКС, в основу которой положен принцип наличия структурных и пусковых механизмов злокачественных аритмий. В зависимости от степени риска ВКС у больных использованы дифференцированные походы к профилактике.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To investigate sudden cardiac death (SCD) predictors in patients after myocardial infarction (MI), to develop the strategy of risk stratification and SCD prevention.</p></sec><sec><title>Material and methods</title><p>Material and methods. In total, 420 patients were examined at Day 10-14 after MI; follow-up period lasted for 1-4 years. General clinical examination, echocardiography, 24-hour electrocardiography (ECG) monitoring, late ventricular potentials (LVP) detection, active orthostatic test (AOT), heart rate variability (HRV) assessment at baseline and during functional tests, as well as psychological testing, were performed. Some participants underwent coronarography and endocardial electrophysiological examination. SCD risk was assessed by Cox multi-factor analysis and Kaplan-Meier survival curve method.</p></sec><sec><title>Results</title><p>Results. According to Cox multivariate regression analysis, the most important predictors included the following six parameters: LVP, HRV reduction, left ventricular ejection fraction &lt;40 %, ventricular arrhythmias, previous MI, and hypotension in AOT Prognostic accuracy and positive predictive value of this model were high enough to assess SCD risk. Based on modeling results, in particular, on the presence of structural and trigger mechanisms of life-threatening arrhythmias, SCD risk stratification was performed. Differential preventive strategy was based on risk levels.</p></sec><sec><title>Conclusion</title><p>Conclusion.
