Изучить функциональное состояние почек и прогностическое значение уровня креатинина и скорости клубочковой фильтрации (СКФ) для летальности больных при острой декомпенсации хронической сердечной недостаточности (ОДХСН).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 392 больных, госпитализированных с клиникой ОДХСН, средний возраст - 65,2+8,4 лет, из них 263 (68 %) мужчин. Длительность ХСН - 3,6+0,5 лет. Содержание сывороточного креатинина определяли ежедневно до 7 дня госпитализации. Стойкое повышение содержания креатинина за время госпитализации &gt; 26,5 мкмоль/л указывало на ухудшение функции почек. СКФ рассчитывалась по формуле Cockroft-Gault.</p></sec><sec><title>Результаты</title><p>Результаты. Исходное нарушение функции почек (СКФ &lt;90 мл/мин/1,73 м2) обнаружено у 270 (69 %) больных, госпитализированных с ОДХСН, при этом тяжелая степень почечной недостаточности (ПН) - СКФ &lt;30 мл/мин/1,73 м2, выявлена у 54 (20 %), из них у 11 (4 %) имела место терминальная стадия ПН - СКФ &lt; 15 мл/мин/1,73 м2. Ухудшение функции почек за время госпитализации отмечено у 120 (30,6 %) больных, и оно было связано в основном с длительностью ХСН. В группе больных с ухудшением функции почек за время госпитализации умерли 13 (10,8 %) пациентов, что было достоверно выше (р&lt;0,05) чем в группе больных без прогрессирования почечной дисфункции, в которой смерть наступила у 19 (7 %) пациентов.</p></sec><sec><title>Заключение</title><p>Заключение. У больных, госпитализированных по поводу ОДХСН, исходно сниженная СКФ обнаружена у 69 % пациентов, при этом в 20 % случаев отмечены тяжелые нарушения функции почек (СКФ &lt;30 мл/мин/1,73 м2). Прогрессирование почечной дисфункции за время госпитализации произошло у 30,6 % больных с ОДХСН, связано оно в основном с длительностью ХСН и сопровождалось более высокой летальностью.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study renal function and prognostic value of creatinine and glomerular filtration rate (GFR) levels in regard to lethality among patients with acute decompensation of chronic heart failure (AD CHF).</p></sec><sec><title>Material and methods</title><p>Material and methods. Fhe study included 392 patients hospitalized with AD CHF symptoms (mean age 65,2+8,4 years; 263 (68%) males; CHF duration - 3,6 years). Serum creatinine concentration was measured daily up to Day 7 of hospitalization. Persistently elevated creatinine level by at least 26,5 mkmol/1 pointed to deteriorating renal function. GFR was calculated according to Cockroft-Gault formula. Echocardiography at admission was performed in all participants.</p></sec><sec><title>Results</title><p>Results. Baseline renal dysfunction (GFR&lt;90 ml/min/1,73 m2) was observed in 270 (69%) patients, hospitalized with AD CHF. Severe renal failure (RF) - GFR&lt;30 ml/min/1,73 m2, was observed in 54 (20%) subjects, including 11 (4%) with terminal RF - GFR&lt;15 ml/min/1,73 m2. Deterioration of renal function during hospitalization was registered in 120 (30,6%) patients, being linked mostly to CHF duration. In this group, 13 (10,8%) patients died - substantially more than in participants without progressing renal dysfunction (19 (7%) deaths).</p></sec><sec><title>Conclusion</title><p>Conclusion. In patients hospitalized with AD CHF, baseline GFR decrease was observed in 69%, including 20% with severe renal dysfunction (GFR&lt;30 ml/min/1,73 m2).
