Оценить частоту сочетания анемии с дисфункцией почек у пациентов с тяжелой хронической сердечной недостаточностью (ХСН) и эффекты терапии внутривенными (в/в) препаратами железа у данной группы больных.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Были обследованы 42 пациента с ХСН III-IV функциональных классов NYHA в возрасте 44-83 лет (средний возраст 69,3+1,2) со стабильным течением ХСН. Скорость клубочковой фильтрации (СКФ) рассчитывали, учитывая концентрацию креатинина по формуле MDRD. Анемию диагностировали по критериям ВОЗ при уровне гемоглобина (НЬ) &lt; 130 г/л у мужчин и &lt; 120 г/л у женщин. 11 пациентов получали терапию Венофером в течение 24 недель, до терапии и после лечения выполняли тест с 6-минутной ходьбой (бмх).</p></sec><sec><title>Результаты</title><p>Результаты. Анемия выявлена у 17 (40,5 %) пациентов, из них 13 имели дефицит железа. Хроническая почечная недостаточность (ХПН) (СКФ &lt; 60 мл/мин) диагностирована в 64,7 % случаев. Был проведен сравнительный анализ клинико-лабораторных показателей в группе пациентов с кардиоренальным синдромом (СКФ &lt; 60 мл/ мин) в зависимости от наличия или отсутствия анемии. Получена достоверная обратная корреляционная связь между уровнем НЬ и креатинина (р=-0,02) и прямая зависимость между уровнем гематокрита (Hi) и СКФ (р=0,044). На фоне терапии в/в препаратами железа отмечено достоверное повышение НЬ со 128,2±14,4г/л до 139,0+17,4 (р=0,03) и Ht с 38,4+3,5 до 41,3+5,06 (р=0,03), и увеличение толерантности к физическим нагрузкам (ТФН) по результатам теста бмх.</p></sec><sec><title>Заключение</title><p>Заключение. Для больных с тяжелой ХСН было характерно сочетание кардиальной патологии с дисфункцией почек и железодефицитной анемией. На фоне терапии препаратами железа в наблюдаемой группе не отмечено серьезных нежелательных явлений, при этом достоверное повышение НЬ и Ht сочеталось с повышением ТФН</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To investigate the prevalence of anemia and renal dysfunction combination among patients with severe chronic heart failure (CHF), as well as the effects of intravenous (i/v) iron therapy.</p></sec><sec><title>Material and methods</title><p>Material and methods. In total, 42 patients (mean age 69,3+1,2 years) with Functional Class III-IV stable CHF (NYHA classification) were examined. Glomerular filtration rate (GFR) was calculated based on creatinine concentration (MDRD formula). Anemia was diagnosed by WHO criteria: hemoglobin (Hb) level &lt;130 g/1 in men and &lt;120 g/1 in women. Eleven patients were administered Venofer for 24 weeks. At baseline and after the treatment course, 6-minute walk test (6mw) was performed.</p></sec><sec><title>Results</title><p>Results. Anemia was diagnosed in 17 (40,5%) patients, including 13 with iron deficiency. Chronic renal failure (CRF; GFR&lt; 60 ml/min) was observed in 64,7 % of the participants. Clinical and laboratory parameters were compared in patients with cardiorenal syndrome (GFR&lt;60 ml/min) with or without anemia. Significant negative correlation was observed between Hb and creatinine levels (p=-0,02), with positive correlation between hematocrit (Ht) and GFR levels (p=0,044). Intravenous iron therapy was associated with a significant increase in Hb (from 128,2+14,4 to 139,0+17,4 g/1; p=0,03), Ht (from 38,4+3,5 to 41,3+5,06; p=0,03), and physical stress tolerance (PST), according to 6mw test results.</p></sec><sec><title>Conclusion</title><p>Conclusion. The combination of cardiac pathology, renal dysfunction and iron-deficient anemia was typical of patients with severe CHF.
