Оценить выполнение в реальной клинической практике современных международных и отечественных рекомендаций по первичной и вторичной профилактике и фармакотерапии сердечно-сосудистых заболеваний (ССЗ) у больных сахарным диабетом 2 типа (СД-2) за период 2004-2006гг. в отдельно взятом округе г. Москвы.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведен одномоментный, ретроспективный анализ медицинских карт 1146 пациентов с СД-2, посетивших одну из 3 городских поликлиник. О качестве первичной и вторичной профилактики и фармакотерапии ССЗ судили по степени соответствия современным международным и отечественным стандартам лечения; анализировали: общую информацию о пациенте, основные факторы риска (ФР), сопутствующие заболевания, частоту использования инструментально-лабораторных методов диагностики и их результаты, количество госпитализаций и листов нетрудоспособности, проводимую терапию.</p></sec><sec><title>Результаты</title><p>Результаты. В амбулаторных картах недостаточно полно представлены ФР и рекомендации по их коррекции: потребление алкоголя и курение, отражены всего в 0,53 % и 6,19% случаев, соответственно. Рекомендации по поводу алкоголя присутствуют в 0,70 %, курения - в 1,83 %, употребления соли - в 2,44% случаев. Рекомендации по контролю за массой тела имели место только в 1,05 % случаев. Выявлено несоответствие между уровнем артериального давления (АД), показателями липидов, глюкозы крови натощак, гликозилированного гемоглобина (HbAlc) и целевыми значениями этих параметров. Целевые значения АД достигнуты у 4,36%, общего холестерина (ОХС) - у 9,68%, ХС липопротеинов низкой плотности - у 18,35 %, глюкозы крови натощак - у 27,67 %, HbAlc - у 24,80 % пациентов с СД-2.</p></sec><sec><title>Заключение</title><p>Заключение. На этапе амбулаторного лечения обнаружено несоответствие между существующими международными и отечественными стандартами профилактики и лечения ССЗ при СД-2 и их реализацией в реальной клинической практике.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the real-world clinical practice implementation of modern international and national recommendations on primary and secondary prevention and pharmacotherapy of cardiovascular disease (CVD) among patients with Type 2 diabetes mellitus (DM-2) in one administrative area of Moscow City (2004-2006).</p></sec><sec><title>Material and methods</title><p>Material and methods. Cross-sectional retrospective analysis of ambulatory medical card data was performed among 1146 DM-2 patients attending one out of three Moscow City South-West Area policlinics (2004-2006). Quality of primary and secondary prevention and pharmacotherapy was assessed by compliance with modern international and national standards. The following data from ambulatory medical cards were analyzed: general information on the patient, main risk factors (RFs), comorbidities, instrumental and laboratory tests and their results, number of hospitalizations and sick-leaves, administered therapy.</p></sec><sec><title>Results</title><p>Results. Ambulatory cards contained inadequate information on RFs and recommendations on RF correction. Alcohol consumption and smoking were mentioned only in 0,53 % and 6,19 % of the cases, respectively. Recommendations on reducing alcohol consumption were recorded for 0,70 %, smoking cessation - for 1,83 %, and salt intake reduction - for 2,44 %. Body weight control was recommended in 1,05 % of the cases only, despite overweight and obesity prevalence of 30 % and &gt;45 %, respectively. The levels of blood pressure (BP), lipids, fasting glucose, glycated hemoglobin (HbAlc) were higher than respective target levels stated in national and international recommendations. Target level of BP was achieved in 4,36 % of DM-2 patients, low-density lipoprotein cholesterol - in 18,35 %, fasting glucose - in 27,67 %, and HbAlc - in 24,80 %.</p></sec><sec><title>Conclusion</title><p>Conclusion.
