Оценить эффективность терапии ивабрадином у пациентов с хронической сердечной недостаточностью (ХСН) III функционального класса (ФК) в плане оценки его влияния на регуляторно-адаптивный статус (РАС). Материал и методы. В исследование были включены 100 пациентов с ХСН III ФК на фоне ишемической болезни сердца (ИБС) и/или гипертонической болезни (ГБ) III стадии, которым назначалась комплексная терапия. После рандомизации первую группу (I гр.) составили 56 пациентов (возраст 62,9±1,8 лет), где был назначен метопролола сукцинат замедленного высвобождения в дозе 59,1±4,5 мг/сут. Во вторую гр. (II гр.) – 44 пациента (возраст 59,4±1,3 лет), которым назначали ингибитор If каналов ивабрадин в дозе 12,1±2,3 мг/сут. в случаях, когда использование β-адреноблокаторов (β-АБ) оказалось невозможным. Исходно и через 6 мес. проводились: тредмилометрия с оценкой максимального потребления кислорода (VO2max) при нагрузке (ФН), эхокардиография, суточное мониторирование артериального давления, определение N-концевого предшественника мозгового натрийуретического пептида (NT-proBNP) плазмы крови. Для объективного количественного определения РАС выполняли пробу сердечно-дыхательного синхронизма. Результаты. Терапия с применением ивабрадина улучшала структурное и функциональное состояние миокарда, повышала толерантность к ФН, вызывала позитивное влияние на содержание NT-proBNP плазмы крови и VO2мах при ФН, в большей степени положительно влияла на РАС. Заключение. Ивабрадин, вероятно, может служить альтернативой β-АБ при невозможности их применения у пациентов с ХСН III ФК на фоне ИБС и/или ГБ III стадии.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the effectiveness of ivabradine-including therapy, as well as the effects of ivabradine on regulatoryadaptive status (RAS), in patients with chronic heart failure (CHF), Functional Class (FC) III. Material and methods. The study included 100 patients with FC III CHF and coronary heart disease (CHD) and/ or Stage III arterial hypertension (AH). The participants were previously prescribed a complex treatment regime. After randomisation, Group I included 56 patients (mean age 62,9±1,8 years), who were additionally administered metoprolol succinate extended-release (mean dose 59,1±4,5 mg/day). Group II (n=44; mean age 59,4±1,3 years) was additionally administered an If channel inhibitor ivabradine, when beta-adrenoblocker (BAB) therapy was not possible. At baseline and 6 months later, participants underwent treadmill test (with VO2max assessment), echocardiography, 24-hour blood pressure monitoring, and the measurement of plasma levels of N-terminal probrain natriuretic peptide (NT-proBNP). RAS status was qualitatively assessed in a cardio-respiratory synchronism test. Results. Ivabradine-including therapy improved myocardial structure and function, increased exercise capacity, and demonstrated positive effects on plasma NT-proBNP levels, VO2мах during treadmill test, and RAS status.</p></sec><sec><title>Conclusion</title><p>Conclusion.
