Изучить возможные подходы к лечению пациентов с ишемической болезнью сердца (ИБС) и гиперхолестеринемией (ГХС), получающих статины, но не достигаюших целевых значений общего холестерина (ОХС) и холестерина липопротеинов низкой плотности (ХС ЛНП) в рамках повседневной клинической практики. Материал и методы. Проект “Treat To Goal” — наблюдательное исследование, в котором приняли участие 10 клинических центров в 6 городах РФ. В исследование были включены 712 пациентов с ИБС + ГХС (386 мужчин и 324 женщин, средний возраст 59 лет), получающих статины с целью вторичной профилактики, но не достигающих целевых значений ОХС и ХС ЛНП. Результаты. Модификация гиполипидемической терапии с целью достижения целевых уровней ОХС осуществлялась следующим образом: у 42,4 % пациентов предпочтение было отдано новому статину; у 32,1 % — проводилось титрование дозы “старого” статина; у 25,5 % пациентов к терапии статинами был добавлен эзетимиб. Там, где проводилось титрование дозы статина, целевого уровня ОХС достигли лишь 33,5 % пациентов, при назначении нового статина — 37,4 %, а при добавлении эзетимиба — 50,4 %. Все три вида модификации терапии характеризовались хорошей переносимостью и безопасностью. Заключение. Комбинированная гиполипидемическая терапия эзетимибом и статином представляет собой наиболее рациональный вариант лечения пациентов с ИБС + ГХС, у которых обычными средствами не удается добиться достижения целевых значений липидных показателей.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study potential therapeutic approaches to the management of coronary heart disease (CHD) patients with hypercholesterolemia (HCH), who receive statins, but do not achieve target levels of total cholesterol (TCH) and low-density lipoprotein cholesterol (LDL-CH) in daily clinical practice. Material and methods. The research project “Treat To Goal” is an observational study, involving 10 clinical centres in 6 Russian cities. In total, the study included 712 patients with CHD and HCH (386 men and 324 women; mean age 59 years), who received statins as secondary prevention measure, but did not achieve target levels of TCH and LDL-CH. Results. In order to achieve target TCH levels, the lipid-lowering therapy was modified as follows: 42,4 % patients were administered a new statin; in 32,1 %, the “old” statin dose was titrated; in 25,5 %, statin therapy was combined with ezetimibe. The target TCH levels were achieved, respectively, in 33,5 %, 37,4 %, and 50,4 % of the patients. All three types of therapy modification were well tolerated and safe.</p></sec><sec><title>Conclusion</title><p>Conclusion.
