Изучить эффекты антагониста рецепторов ангиотензина II (АРА II) валсартана и статина (флувастатина XL) в монотерапии и комбинации у пациентов с артериальной гипертонией (АГ) и дислипидемией (ДЛП) в отношении эндотелиальной функции (ЭФ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. 30 пациентов с неосложненной, ранее не леченной, эссенциальной АГ I-II ст. и ДЛП Па и Пб типов были рандомизированы для 8-недельной монотерапии валсартаном или флувастатином XL с последующим добавлением второго компонента — флувастатина XL или валсартана, соответственно.</p></sec><sec><title>Результаты</title><p>Результаты. На фоне монотерапии валсартаном отмечено снижение прироста линейной скорости кровотока (ЛСК) в пробе с реактивной гиперемией (РГ) — 177,7+5,8 vs 170,0+4,9% (p&lt;0,01); эндотелий-зависимая вазодилатация плечевой артерии (ЭЗВД ПА) в пробе с РГ увеличилась на 14% (p&gt;0,05). На фоне монотерапии флувастатином XL 80 мг/сут. в течение 8 нед. произошло незначительное (на 2%), но достоверное снижение систолического артериального давления (САД) при отсутствии динамики диастолического АД, улучшение ЭФ по степени прироста ЭЗВД ПА при отсутствии изменений прироста ЛСК в пробе с РГ. Продолжение комбинированной терапии приводило к дальнейшему улучшению ЭФ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the effects of an angiotensin II receptor antagonist (ARA II) valsartan, a statin (fluvastatin XL), and their combination on endothelial function (EF) in patients with arterial hypertension (AH) and dyslipidemia (DLP).</p></sec><sec><title>Material and methods</title><p>Material and methods. In total, 30 patients with untreated, non-complicated Stage I-II AH and DLP IIA or IIB were randomised for 8-week monotherapy with valsartan or fluvastatin XL, with the subsequent addition of the second component, fluvastatin XL or valsartan, respectively.</p></sec><sec><title>Results</title><p>Results. Valsartan monotherapy was associated with a reduced increase in linear blood flow velocity (LFV) during the reactive hyperemia test (RHT) - 177,7+5,8 vs. 170,0+4,9% (p&lt;0,01). At the same time, brachial artery endothelium-dependent vasodilatation (BA EDVD) increased by 14% (p&gt;0,05). Eight-week monotherapy with fluvastatin XL (80 mg/d) was linked to moderate but statistically significant reduction (-2%) in systolic blood pres­sure (SBP), with no effect on diastolic BP level. In RHT, LFV increase did not change, but EF improved, as manifested in BA EDVD increase. Prolonged combined therapy resulted in further EF improvement.</p></sec><sec><title>Conclusion</title><p>Conclusion.
