Изучить возможности использования доказательных клинико-морфологических критериев целесообразности чрескожных коронарных вмешательств (ЧКВ) для экспертизы применения высокотехнологичных процедур коронарной реваскуляризации в российской популяции больных с острым коронарным синдромом (ОКС).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Оценивали обоснованность выполненного ЧКВ у пациентов с ОКС, перенесших коронарную реваскуляризацию, и определяли потенциальную потребность в ЧКВ среди пациентов, не подвергавшихся ЧКВ на коронарных артериях. Оценка проводилась с помощью клинико-морфологических критериев, изложенных в ACCF/SCAI/STS/AATS/AHA/ASNC/HFSA/SCCT 2012 Appropriate Use Criteria for Coronary Revascularization Focused Update (далее — ACCF 2012). Анализировали данные 65912 больных с ОКС за 2010–2011 гг. Источник данных — российский регистр ОКС (организован РКНПК, г. Москва).</p></sec><sec><title>Результаты</title><p>Результаты. Критерии ACCF 2012 позволили вынести суждение о клинической обоснованности оказанной высокотехнологичной медицинской помощи у 79,2% пациентов, получивших ЧКВ, а также определить потенциальную потребность в ЧКВ у 80,6% пациентов, кому вмешательство выполнено не было. Среди пациентов с ОКС, кому выполнено ЧКВ (n=9147), в 68,9% случаев вмешательство было обоснованно. Доля необоснованно выполненных ЧКВ составила 4,6%. Среди пациентов, кому вмешательство не проводилось (n=56765), оно было бы целесообразно в 57,9% случаев.</p></sec><sec><title>Заключение</title><p>Заключение. Клинико-морфологические критерии ACCF 2012 позволяют судить об обоснованности выполнявшейся ЧКВ и определить потенциальную потребность в ЧКВ у большинства больных с ОКС российской популяции. У значительной части больных с ОКС проведение ЧКВ было обосновано. Исследование показало возможность применения доказательных клинико-морфологических критериев для экспертизы применения высокотехнологичной помощи у российских больных с ОКС.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study possibility of using the evidence-based clinical-andmorphological appropriate use criteria for percutaneous coronary interventions (PCIs) for expert evaluation of high-technology procedures implementation in patients with acute coronary syndrome (ACS) in Russia.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The appropriateness of performed PCI was assessed in patients with ACS, underwent coronary revascularization. The potential need in PCI was determined in ACS patients refused from coronary revascularization. Assessment was performed with the help of ACCF/SCAI/STS/AATS/AHA/ASNC/HFSA/SCCT 2012 Appropriate Use Criteria for Coronary Revascularization Focused Update (ACCF 2012). Data from 65,912 ACS patients, containing in Russian ACS Registry (2010–2011) were examined.</p></sec><sec><title>Results</title><p>Results. ACCF 2012 criteria allow to assess the clinical appropriateness of PCI in 79.2% of patients underwent coronary revascularization and to determine the potential need in PCI in 80.6% of patients, refrained from coronary revascularization. Among ACS patients underwent PCI (n=9147), intervention was appropriate in 68.9% of cases. Inappropriate PCI was revealed in 4.6% of cases. Among patients refrained from PCI (n=56765), coronary revascularization was potentially appropriate in 57.9% of cases.</p></sec><sec><title>Conclusion</title><p>Conclusion. ACCF 2012 clinical-and-morphological criteria allow to judge on appropriateness of performed PCI and to evaluate the potential need in PCI among the most part of Russian ACS patients. In present study coronary revascularization was appropriate in the majority of ACS patients.
