Оценить взаимосвязи функциональной активности эндотелия сосудов и показателей суточного профиля (СП) АД у больных с неконтролируемой в амбулаторных условиях артериальной гипертензией (АГ) 1-2 ст. на фоне терапии кандесартаном и лозартаном. Материал и методы. В исследование включены 60 больных в возрасте 35-65 лет, из которых были сформированы 2 группы (гр.). I гр. – 31 больной АГ, из них 15 мужчин и 16 женщин, средний возраст 46,1±7,1 лет, принимающих в качестве антигипертензивной терапии (АГТ) лозартан. Во II гр. Включены 29 больных: 12 мужчин и 17 женщин, средний возраст 47,2±6,8 лет, принимающих в качестве АГТ кандесартан. Проводили суточное мониторирование (СМ) АД и определение уровня стабильных конечных метаболитов оксида азота (NOх) в сыворотке крови, в качестве показателей функциональной активности эндотелия сосудов. Результаты. При сравнительном анализе препаратов по влиянию на СП АД получено достоверное преимущество кандесартана над лозартаном по ряду показателей СМАД. При лечении кандесартаном выявлена достоверная положительная динамика в виде увеличения уровня NOx в сыворотке крови с 38,56±4,68 мкМ/л до 89,17±10,5 мкМ/л (p&lt;0,0001). В гр. лозартана обнаружена достоверная положительная динамика в виде увеличения уровня NOx в сыворотке крови с 37,19±2,43 мкМ/л до 59,32±7,03 мкМ/л (p&lt;0,001). При корреляционном анализе получены данные о достоверной взаимосвязи уровня NO в сыворотке крови и основных показателей СП диастолического АД исходно и на фоне лечения кандесартаном. Аналогично, как в случае с кандесартаном, в гр. лозартана была обнаружена обратная корреляционная связь между содержанием NOх и показателями СМАД. Заключение. Кандесартан обладает достоверным преимуществом над лозартаном по влиянию на СП АД и активность эндотелиальной функции.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To assess the associations between endothelial functional activity and circadian blood pressure profile (CBPP) in hypertensive patients (Stage I-II arterial hypertension, uncontrolled in ambulatory settings) who were administered candesartan and losartan. Material and methods. The study included 60 patients, aged 35-65 years. All participants were divided into two groups: Group I (n=31; 15 men, 16 women; mean age 46,1±7,1 years) receiving losartan, and Group II (n=29; 12 men, 17 women; mean age 47,2±6,8 years) receiving candesartan. The examination included 24-hour BP monitoring (BPM) and the measurement of serum levels of nitric oxide (NO) stable end products, as the markers of endothelial functional activity. Results. Candesartan demonstrated a more pronounced beneficial effect on several 24-hour BPM parameters, compared to losartan. The candesartan group showed a significant increase in serum NO (NOx) levels (from 38,56±4,68 to 89,17±10,5 μmol/l; p&lt;0,0001). In the losartan group, serum NO (NOx) levels increased from 37,19±2,43 to 59,32±7,03 μmol/l (p&lt;0,001). Serum NO levels significantly correlated with the main CBPP parameters for diastolic BP, both at baseline and during the candesartan treatment. Similarly, the losartan group demonstrated a significant negative correlation between NO levels and 24-hour BPM parameters. Conclusion.
