Оценить эффективность комбинированной терапии ивабрадином и β-адреноблокаторами (β-АБ) в сравнении с тактикой титрования доз β-АБ. Материал и методы. В многоцентровое, открытое, контролируемое исследование включены 1104 пациента со стабильной стенокардией (СтС) II-III функционального класса (ФК), синусовым ритмом при частоте сердечных сокращений (ЧСС)≥60 уд/мин на фоне регулярной терапии β-АБ, доза которого не должна быть максимальной. Больных включали в группу (гр.) «обычной» терапии (терапия β-АБ с титрацией дозы до максимально переносимой) – 228 (20,7%) пациентов и в гр. ивабрадина – 876 (79,3%) больных. Результаты. Исходно гр. не различались по основным клинико-демографическим характеристикам. Добавление ивабрадина к стабильной терапии β-АБ привело к более существенному снижению ЧСС, чем в гр. обычной терапии к 16 нед. лечения – 61±6 и 63±8 уд/мин (p=0,001). К окончанию лечения в гр. ивабрадина было достоверно большее количество пациентов с I ФК стенокардии – 37,1% vs 28% (p=0,017). При учете приступов стенокардии за период с 8 по 16 нед. в гр. ивабрадина было отмечено существенно меньшее число приступов стенокардии по сравнению с гр. обычной терапии – 4 (2;10) vs 6 (2;15), соответственно (p=0,015). В гр. обычной терапии достоверно чаще в сравнении с гр. Ивабрадина отмечались побочные эффекты (ПЭ) в виде приступов удушья, одышки, гипотонии и утомляемости. В целом в гр. ивабрадина отмечено в 2 раза меньше ПЭ, чем в гр. обычной терапии – 18,4% и 9,4%, соответственно (p&lt;0,001). Заключение. Комбинированная терапия β-АБ и ивабрадином отличалась высокой безопасностью, а также выраженным антиангинальным действием, что привело к значительному улучшению клинического состояния больных СтС по сравнению с тактикой титрации доз β-АБ.</p></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To compare the effectiveness of the combination therapy with ivabradine and β-adrenoblockers (β-AB) and the dose-titration β-AB therapy. Material and methods. This multi-centre, open, controlled study included 1104 patients with functional Class (FC) II-III stable angina (SA), sinus rhythm, and heart rate (HR) ≥60 bpm, who received regular β-AB therapy in submaximal doses. The participants were divided into the standard therapy group (β-AB therapy with the dose titration to the maximal tolerated dose; 228 patients (20,7%)) and the ivabradine therapy group (876 patients (79,3%))</p></sec><sec><title>Results</title><p>Results. Both groups were comparable by main demographical and clinical characteristics. Adding ivabradine to the β-AB therapy resulted in a more pronounced HR reduction at Week 16, compared to the standard therapy group (61±6 vs. 63±8 bpm; p=0,001). By the end of the study, the percentage of the patients with FC I SA was significantly higher in the ivabradine group than in the standard therapy group (37,1% vs. 28%; p=0,017). The average number of angina attacks between Weeks 8 and 16 was significantly lower in the ivabradine group than in the standard therapy group – 4 (2;10) vs. 6 (2;15), respectively (p=0,015). In the standard therapy group, the incidence of adverse events, such as dyspnoea, hypotension, and fatigue, was significantly higher than in the ivabradine group – 18,4% vs. 9,4%, respectively (p&lt;0,001). Conclusion.
