Изучить варианты ремоделирования левого желудочка (ЛЖ) у мужчин 18-25 лет с и без артериальной гипертонии (АГ). Материалы и методы. Обследованы 211 мужчин 18-25 лет (средний возраст 21,1±0,14) с анамнезом повышения артериального давления (АД) при случайных клинических измерениях. Выполнены повторные измерения клинического АД, суточное мониторирование АД (СМАД), эхокардиография с расчетом индекса массы миокарда левого желудочка (ИММЛЖ), относительной толщины стенок (ОТС) и исследованием диастолической функции (ДФ) ЛЖ. Результаты. При сопоставлении результатов клинического измерения и СМАД нормальный уровень АД был у 36 (17 %), устойчивая АГ — у 124 (58,7 %), неустойчивая АГ — у 51 (24,3 %). У пациентов с устойчивой АГ ИММЛЖ достоверно был больше, чем у нормотоников. У лиц с устойчивой АГ у 43 (34,7 %) пациентов выявлена нормальная геометрия ЛЖ, концентрическое ремоделирование у 75 (60,5 %), у 6 (4,8) — концентрическая гипертрофия ЛЖ. Среди пациентов с неустойчивой АГ у 14 (27,5 %) имело место концентрическое ремоделирование. При исследовании ДФ ЛЖ у пациентов с устойчивой и неустойчивой АГ скоростной параметр трансмитрального кровотока Е/А был меньше, а скорость волны А достоверно больше, чем у нормотоников. Заключение. У молодых мужчин в возрасте 18-25 лет с устойчивой АГ преобладающим типом ремоделирования является концентрическое ремоделирование ЛЖ с тенденцией к формированию диастолической дисфункции ЛЖ.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To investigate the variants of left ventricular (LV) remodelling in men aged 18-25 years, with and without arterial hypertension (AH). Material and methods. In total, the study included 211 men aged 18-25 years (mean age 21,1±0,14 years), with random blood pressure (BP) elevation at previous clinical examinations. All participants underwent repeat BP measurements, 24-hour BP monitoring (BPM), echocardiography with calculation of LV myocardial mass index (LVMMI) and relative wall thickness (RWT), and evaluation of LV diastolic function (DF). Results. According to the results of clinical BP measurements and 24-hour BPM, normal BP levels were observed in 36 participants (17 %), stable AH — in 124 (58,7 %), and unstable AH — in 51 (24,3 %). In patients with stable AH, LVMMI was significantly higher than in individuals with normal BP. Among participants with stable AH, 43 (34,7 %) had normal LV geometry, 75 (60,5 %) had concentric LV remodelling, and 6 (4,8 %) — concentric LV hypertrophy. Among participants with unstable AH, 14 (27,5 %) had concentric LV remodelling. Comparing to normotensive individuals, patients with stable and unstable AH had lower transmitral flow ratio E/A, and significantly higher peak A velocity. Conclusion.
