Изучение прогностической значимости массы миокарда левого желудочка (ММ ЛЖ) при бессимптомном аортальном стенозе (АС) в длительном амбулаторном исследовании, а также влияния показателя ММ ЛЖ на основные сердечно-сосудистые события (ССС).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Регрессионный анализ Кокса использовали для оценки влияния эхокардиографически выявленной увеличенной ММ ЛЖ на частоту основных ССС: серьезное кардиоваскулярное событие; события, связанные с аортальным клапаном — комбинированная замена аортального клапана, застойная сердечная недостаточность вследствие АС, и ишемические кардиоваскулярные события — нефатальный инфаркт миокарда, негемморагиче-ский инсульт, коронарная реваскуляризация, госпитализация из-за нестабильной стенокардии, у 165 пациентов; средний возраст 67 лет; 39,6% женщин с бессимптомной формой легкого-умеренного АС. Пациенты отобраны с бессимптомным, в основном, умеренным АС, без диабета, кардиоваскулярной или почечной патологии. Пациентов наблюдали в течение 4 лет.</p></sec><sec><title>Результаты</title><p>Результаты. В начале исследования индекс ММ ЛЖ (ИММ ЛЖ) составлял 45,9±14,9 г/м2, а максимальная скорость струи в аорте 3,09±0,54 м/с. Во время наблюдения произошло 58 основных ССС. В регрессионном анализе Кокса 1 SD — 1 стандартное отклонение — (15 г/м2) выше исходного уровня ИММ ЛЖ определяет прогнозируемое увеличение риска на 12% для основных ССС, 28% для ишемических ССС, 34% для сердечно-сосудистой смертности и 23% для общей смертности и госпитализации по поводу сердечной недостаточности (все р&lt;0,01), вне зависимости от искажающих результаты факторов. В изменяющихся во времени моделях, принимая прогрессивное увеличение индекса массы миокарда ЛЖ во время наблюдения — 1 SD — более высокий ИММ ЛЖ в исследовании был последовательно связан с возрастанием с 13% до 61% риска для ССС (все р&lt;0,01), независимо от возраста, пола, индекса массы тела, фракции выброса ЛЖ и концентричности, а также наличия сопутствующей гипертонии.</p></sec><sec><title>Заключение</title><p>Заключение. Более высокий ИММ ЛЖ независимо связан с повышенной сердечно-сосудистой заболеваемостью и смертностью при прогрессировании АС.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the prognostic value of Left Ventricular Mass (LVM) with asymptomatic aortic stenosis (AS) in a long-term outpatient study, as well as the effect of the LVM on major cardiovascular events (CVe).</p></sec><sec><title>Material and methods</title><p>Material and methods. Cox regression analysis was used to evaluate the effect of an echocardiographically detected increased LVM on the frequency of major cardiovascular diseases: a serious cardiovascular event; events related to the aortic valve — combined aortic valve replacement, congestive heart failure due to AS, and ischemic cardiovascular events — nonfatal myocardial infarction, non-hemorrhagic stroke, coronary revascularization, hospitalization due to unstable angina. The study included 165 patients with average age of 67 years; 39,6% of women had asymptomatic form of mild and moderate AS. Patients were selected with asymptomatic, mainly moderate AS, without diabetes, cardiovascular or renal pathologies. Patients were observed for 4 years.</p></sec><sec><title>Results</title><p>Results. At the beginning of the study, the LVM index was 45,9±14,9 g/ m2, and the peak velocity in the aorta was 3,9±0,54 m/s. During the observation, 58 major CVE were indentified. In Cox regression analysis, 1 standard deviation (15 g/m2) above the initial level of the LVM index determines the predicted increase in risk by 12% for major CVE, 28% for ischemic CVE, 34% for cardiovascular mortality and 23% for all cause mortality and hospitalization for heart failure (all p&lt;0,01). A higher LVM index was consistently associated with CVE risk increase from 13% to 61% (all p&lt;0,01) regardless of age, gender, body mass index, LV ejection fraction, concentricity and presence of concomitant hypertension.</p></sec><sec><title>Conclusion</title><p>Conclusion.
