Оценить влияние структурной перестройки артерий почек малого диаметра на вероятность развития тубулоинтерстициального фиброза у пациентов с хроническим гломерулонефритом.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследовании приняли участие 97 пациентов с первичными гломерулонефритами. Средний возраст пациентов составил 37,27±1,2 лет. Критерием включения пациентов в исследование являлось наличие показаний к проведению нефробиопсии. При нефробиопсии производился анализ состояния почечной ткани, в частности наличие или отсутствие тубулоинтерстициального фиброза (ТИФ). На основании наличия или отсутствия признака ТИФ пациенты были ранжированы в 2 группы. К первой группе отнесены пациенты с ТИФ, ко второй — без ТИФ. В рамках нефробиопсии была проведена вазометрия — измерение структур почечных артерий малого диаметра. Учитывались показатели внутреннего и внешнего диаметров, толщина стенки артерии, интимы, медии, комплекса интима-медиа (КИМ), сосудистая масса.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено влияние размера внутреннего и внешнего диаметров междольковой артерии (МА) на изучаемый риск. Необходимо отметить, что вероятность развития ТИФ повышается при уменьшении внутреннего диаметра МА (χ2-критерий =5,34, р=0,02) и увеличении внешнего диаметра МА (χ2-критерий =7,77, р=0,005). Толщина интимы не имеет статистически достоверного влияния на риск формирования ТИФ (р=0,29), тогда как толщина медии имеет высоко достоверное статистическое значение в ремоделировании почечной ткани (χ2-критерий =13,71, р=0,0002). Установлено статистически достоверное влияние увеличения сосудистой массы на риск ТИФ (χ2-критерий =6,55, р=0,01).</p></sec><sec><title>Заключение</title><p>Заключение. Установлено, что изменение структуры артерий почек малого диаметра при хроническом гломерулонефрите происходит по гипертрофическому типу. Показано влияние ремоделирования артериального русла малого диаметра на вероятность развития ТИФ, в частности — увеличение внешнего диаметра артерий, медии, сосудистой массы и уменьшение внутреннего диаметра.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the role of remodeling of small diameter kidney arteries in the prognosis of progression of tubulointerstitial fibrosis in patients with chronic glomerulonephritis.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study involved 97 patients with primary glomerulonephritis. The average age of patients was 37,27±1,2 years. The inclusion criterion was the presence of indications for renal biopsy. Renal biopsy was used to analyze renal tissue, in particular, for determining of tubulointerstitial fibrosis (TIF). Based on TIF sign, the patients were divided in 2 groups. The first group includes patients with TIF, the second — without TIF. Vasometry, a measurement of the renal arteries structures, was performed as part of renal biopsy. Parameters of internal and external diameters, wall thickness, intima, media, intima-media complex, vascular mass were analyzed.</p></sec><sec><title>Results</title><p>Results. The influence of the size of the inner and outer diameters of the interlobular artery (MA) on the studied risk was established. Probability of TIF developing increases with decreasing of MA inner diameter (χ2-criterion =5,34, p=0,02) and increasing of MA outer diameter (χ2-criterion =7,77, p=0,005). Intimal thickness does not have a statistically significant effect on the risk of TIF developing (p=0,29), whereas media thickness has a high statistical significance in renal tissue remodeling (χ2-criterion =13,71, p=0,0002). A statistically significant effect of an increase in the vascular mass on the risk of TIF was determined (χ2-criterion =6,55, p=0,01).</p></sec><sec><title>Conclusion</title><p>Conclusion. We established that changes in the structure of small diameter kidney arteries in chronic glomerulonephritis occurs according to a hypertrophic type.
