Изучить активность провоспалительных цитокинов у пациентов с ишемической болезнью сердца (ИБС), ассоциированной с синдромом обструктивного апноэ сна (СОАС) в зависимости от характера антиагрегантной терапии.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. С целью оценки провоспалительной активности цитокинов по уровню интерлейкинов (IL): IL- 1β, IL-6, IL-10 на фоне антиагрегантной терапии и ее связи с агрегацией тромбоцитов было обследовано 148 пациентов со стабильной ИБС, из которых легкая степень СОАС у 30% (n=44), среднетяжелая и тяжелая степень у 10% (n=14), пациенты без СОАС составили 60% (n=90).</p></sec><sec><title>Результаты</title><p>Результаты. Агрегационная активность тромбоцитов на фоне двойной антиагрегационной терапии у пациентов с СОАС характеризовалась отсутствием различий по агрегационной активности (спонтанной и 0,1 мкМ АДФ-индуцированной) между пациентами с наличием или отсутствием СОАС, что свидетельствует о синергизме и потенциировании антиагрегантного эффекта клопидогрела и ацетилсалициловой кислоты. Ассоциация ИБС со среднетяжелой и тяжелой формой СОАС сопровождается более высокими уровнями провоспалительных IL- 1β и IL-6 по сравнению с пациентами без СОАС. Антиагрегантная терапия, особенно двойная, сопровождается противовоспалительным эффектом, который отражается более низкими значениями провоспалительных IL как у пациентов с сопутствующим СОАС, так и без синдрома ночной обструкции.</p></sec><sec><title>Заключение</title><p>Заключение. Среднетяжелая и тяжелая формы СОАС у пациентов со стабильной ИБС сопровождается более высокими значениями провоспалительных IL (1β, 6) и агрегации тромбоцитов по сравнению с пациентами без сопутствующего СОАС. Сочетанное применение клопидогрела и ацетилсалициловой кислоты сопровождается потенцированием антиагрегантного и противовоспалительного эффектов, что отражается меньшими значениями провоспалительных IL и более низкой агрегационной активностью тромбоцитов при стабильной ИБС, независимо от сопутствующего СОАС.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the activity of pro-inflammatory cytokines in patients with coronary artery disease (CAD) associated with obstructive sleep apnea (OSA), depending on the type of antiplatelet therapy.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 148 patients with stable CAD, 30% (n=44) of whom had mild OSA, 10% (n=14) — moderate and severe OSA. There were 60% (n=90) of patients without OSA. Pro-inflammatory activity of cytokines was assessed by the level of interleukins (IL): IL- 1β, IL-6, IL-10.</p></sec><sec><title>Results</title><p>Results. Using dual antiplatelet therapy, there were no differences in aggregation activity of platelets (spontaneous and induced by 0,1 pM ADP) in patients with and without OSA. This indicates the synergism and potentiation of the antiplatelet effect of clopidogrel and acetylsalicylic acid (ASA). Patients with moderate and severe OSA and stable CAD had higher levels of IL- 1β and IL-6 compared with patients without OSA. Antiplatelet therapy, especially dual one, had an anti-inflammatory effect, which was reflected in lower values of pro-inflammatory IL both in patients with and without OSA.</p></sec><sec><title>Conclusion</title><p>Conclusion. Patients with moderate and severe OSA and stable CAD had higher levels of pro-inflammatory IL (IL- 1β, IL-6) compared with patients without OSA. The combined use of clopidogrel and ASA potentiates antiplatelet and anti-inflammatory effects.
