Оценить клиническую эффективность катетерного лечения фибрилляции предсердий (ФП) у пациентов с пароксизмальной формой ФП и сопутствующим обструктивным апноэ сна тяжелой и средней степени тяжести с проведением и без проведения СИПАП-терапии (коррекция обструктивных нарушений дыхания во сне с применением устройства для обеспечения положительного давления в верхних дыхательных путях; длительность применения &gt;4 ч в течение ночи).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. 60 пациентов с пароксизмальной формой ФП и обструктивным апноэ сна средней/тяжелой степени, перенесших катетерную изоляцию устьев легочных вен, были рандомизированы в соотношении 2:1 в 2 группы: СИПАП-группа (n=40) и группа контроля (n=20). Срок наблюдения составил 12 мес. с контрольными визитами через 3, 6 и 12 мес. Клиническая эффективность катетерной аблации оценивалась после окончания 3-месячного слепого периода.</p></sec><sec><title>Результаты</title><p>Результаты. Частота свободы от аритмии у пациентов, получающих СИПАП-терапию, составила 82,9% и была достоверно больше при сравнении с группой контроля — 47,5% (p=0,032). Предиктором возврата аритмии являлась сопутствующая гипертоническая болезнь (отношение шансов 24,1; 95% доверительный интервал 2,2261,2).</p></sec><sec><title>Заключение</title><p>Заключение. Коррекция обструктивных нарушений дыхания во сне с применением СИПАП-терапии улучшает клиническую эффективность катетерного лечения пароксизмальной формы ФП в сроки наблюдения 12 мес.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the effectiveness of catheter ablation in paroxysmal atrial fibrillation (AF) in patients with moderate/severe obstructive sleep apnea (OSA) with/without use of continuous positive airway pressure (CPAP) therapy (&gt;4 hours per night).</p></sec><sec><title>Material and methods</title><p>Material and methods. A total of 60 patients with paroxysmal AF and moderate/severe OSA after catheter pulmonary vein isolation were randomized into 2 groups (2:1 ratio): CPAP group (n=40) and control group (n=20). The follow-up period was 12 months and included visits after 3, 6 and 12 months. The effectiveness of catheter ablation was assessed after the 3-month blind period.</p></sec><sec><title>Results</title><p>Results. Freedom from arrhythmia in patients with CPAP therapy was documented in 82,9% and was significantly more common compared with the control group — 47,5% (p=0,032). Concurrent hypertension was a predictor of arrhythmia recurrence (odds ratio 24,1; 95% confidence interval 2,2-261,2).</p></sec><sec><title>Conclusion</title><p>Conclusion.
