Ретроспективно проанализировать особенности поражения сердечно-сосудистой системы у пациентов с морфологически верифицированным системным амилоидозом.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведена ретроспективная оценка 38 случаев системного амилоидоза, среди которых в 57,9% случаев диагностика была осуществлена прижизненно (у 27,3% из них диагноз впоследствии был подтвержден при аутопсии), в 42,1% случаев диагноз установлен на аутопсии. Морфологическая верификация осуществлялась с помощью окраски Конго красным с исследованием в поляризованном свете. Критериями диагностики системного характера поражения являлись морфологическая верификация депозитов амилоида в биоптатах ≥2 локализаций или морфологически верификация депозитов амилоида в биоптате одной локализации в сочетании с клинической картиной поражения, характерной для амилоидоза, как минимум одного иного органа. Медиана (здесь и далее в скобках указаны 25- и 75-процентили) возраста пациентов с системным амилоидозом составила 72,5 (64-84) года. Среди них 65,8% женщин, 34,2% мужчин.</p></sec><sec><title>Результаты</title><p>Результаты. В анамнезе у 81,6% пациентов с системным амилоидозом была задокументирована артериальная гипертония, у 52,6% — ишемическая болезнь сердца, у 34,2% — инфаркт миокарда, у 36,8% — сахарный диабет 2 типа, у 44,7% — фибрилляция предсердий, у 26,3% пациентов — нарушения проводимости. 13,2% пациентов был имплантирован электрокардиостимулятор. На электрокардиограмме у 42,3% пациентов отмечался низкий вольтаж комплекса QRS. По данным эхокардиографии гипертрофия левого желудочка (ЛЖ) выявлена у 94,7% пациентов. У 31,6% пациентов имелись признаки “зернистости” миокарда; у 20% — нарушения локальной сократимости ЛЖ, у 77,8% — повышение давления в легочной артерии; у 28,6% — признаки диастолической дисфункции ЛЖ, у 14,3% — нормальная диастолическая функция. Фракция выброса (ФВ) ЛЖ (по Симпсону) составила 55 (44,5-55)%. ФВ ЛЖ &lt;40% выявлена у 15%, 40-50% у 20%, &gt;50% у 65% пациентов. Хроническая сердечная недостаточность была указана в клиническом диагнозе у 71,1% пациентов с системным амилоидозом, хроническая болезнь почек у 95% пациентов. Расчетная скорость клубочковой фильтрации составила 21,2 (13,1-40) мл/мин/1,73 м2. Протеинурия (1,1 (0,5-3,0) г/л) выявлена у 88,9% пациентов. Нефротический синдром имел место у 27,8% больных.</p></sec><sec><title>Заключение</title><p>Заключение. Больным с поражением сердечно-сосудистой системы неясной этиологии, особенно при наличии утолщения стенки ЛЖ, сохраненной ФВ ЛЖ, хронической болезни почек 4-5 стадий и протеинурии, необходим скрининг на системный амилоидоз.</p></sec><sec><title> </title><p> </p></sec></abstract><trans-abstract xml:lang="en"><p>Amyloidosis is a heterogeneous group of the diseases caused by the extracellular deposition of amyloid in various organs including a cardiovascular system.</p><sec><title>Aim</title><p>Aim. To study the cardiovascular damage in patients with systemic amyloidosis.</p></sec><sec><title>Material and methods</title><p>Material and methods. A retrospective assessment of 38 cases of systemic amyloidosis was carried out. Diagnosis was verified by biopsy in 57,9% (27,3% of these cases were then confirmed by autopsy) and autopsy in 42,1% of patients. Congo red stain and polarized-light microscopy were used. The systemic nature of lesion was diagnosed in the positive biopsies of two or more localizations or in the positive biopsy of one localization in the combination with the clinical picture of amyloidosis at least one organ. Median (interquartile range) of age was 72,5 (64-84) years (women, 65,8%; men, 34,2%).</p></sec><sec><title>Results</title><p>Results. In the medical history, 81,6% of patients with systemic amyloidosis had hypertension, 52,6% — coronary artery disease, 34,2% — myocardial infarction, 36,8% — type 2 diabetes, 44,7% — atrial fibrillation, 26,3% — conduction disorders. Also 13,2% of patients had an implanted pacemaker. According to the electrocardiography, 42,3% of patients had a low QRS voltage. According to echocardiography, left ventricular (LV) hypertrophy was detected in 94,7% of patients. In addition, 31,6% of patients had signs of granular sparkling myocardial appearance, 20% — impaired local LV contractility, 77,8% — increased pulmonary artery reassure, 28,6% — LV diastolic dysfunction, 14,3% — normal diastolic function. LV ejection fraction (LVEF) was 55 (44,5-55)%. LVEF &lt;40% was detected in 15%, 4050% — in 20%, &gt;50% — in 65% of patients. Heart failure was indicated in 71,1% of patients with systemic amyloidosis, chronic kidney disease (CKD) — in 95% of patients. The estimated glomerular filtration rate (CKD-EPI) was 21,2 (13,1-40) ml/min/1,73 m2. Proteinuria (1,1 (0,5-3,0) g/l) was detected in 88,9% of patients. Nephrotic syndrome occurred in 27,8% of patients.</p></sec><sec><title>Conclusion</title><p>Conclusion.
