Изучить базовый уровень маркеров воспаления (МВ) и белков системы протеолиза сыворотки крови у пациентов, направленных на катетерную изоляцию устьев легочных вен (ЛВ), и выявить возможную способность МВ прогнозировать наличие аритмогенных очагов вне муфт ЛВ — невенозависимой фибрилляции предсердий (ФП).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование были включены 100 пациентов с пароксизмальной (n=89) и персистирующей (n=11) формой ФП, которым выполнялась первичная процедура криобаллонной аблации устьев ЛВ с одномоментной имплантацией петлевого регистратора ЭКГ (Reveal XT, Medtronic). Перед процедурой у всех пациентов брали кровь для последующего анализа МВ: N-терминальный фрагмент пропептида мозгового натрийуретического гормона (NTproBNP), матриксные металлопротеиназы 1, 3, 9, тканевой ингибитор матрикс-ной металлопротеиназы-1, фактор роста фибробластов-2, белок, связывающий жирные кислоты-3, трансформирующий фактор роста р-1, фактор некроза опухоли-а, интерлейкин-1р.</p></sec><sec><title>Результаты</title><p>Результаты. Продемонстрировано достоверное негативное прогностическое значение базового NTproBNP (отношение рисков — 1,00053, p=0,00935) на рецидивирование ФП (риск рецидива повышается на 8,4% при повышении уровня NTproBNP на 100 пг/мл). Для остальных параметров достоверного влияния на прогноз рецидива в послеоперационном периоде выявлено не было.</p></sec><sec><title>Заключение</title><p>Заключение. Роль МВ в патогенезе ФП должна изучаться далее с учетом полученных рекомендаций о численности выборки для оценки их предиктивной способности в отношении выявления на дооперационном этапе пациентов с невенозависимой ФП.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the baseline level of inflammatory markers (IM) and proteins of serum proteolytic system in patients scheduled for pulmonary vein isolation, and to analyze the potential of IM to predict the arrhythmogenic foci outside the PV sleeves — non-pulmonary vein dependent atrial fibrillation (AF).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 100 patients with paroxysmal (n=89) and persistent (n=11) AF who underwent the primary PV cryoballoon ablation with the simultaneous implantation of a loop recorder (Reveal XT, Medtronic). Before the procedure, we analyzed following parameters in all patients: N-terminal pro-brain natriuretic peptide (NTproBNP), matrix metalloproteinases 1, 3, 9, tissue inhibitor of matrix metalloproteinase 1, fibroblast growth factor-2, fatty acid binding protein 3, transforming growth factor p1, tumor necrosis factor a, interleukin 1p.</p></sec><sec><title>Results</title><p>Results. A significant negative predictive value of baseline NTproBNP (hazard ratio, 1,00053, p=0,00935) for recurrent AF (recurrence risk increases by 8,4% with an increase in NTproBNP level by 100 pg/ml) was demonstrated. For the rest of the parameters, there was no significant effect on recurrence risk in the postoperative period.</p></sec><sec><title>Conclusion</title><p>Conclusion.
