Шкалы риска, основанные на традиционных факторах риска, не позволяют считать вопрос стратификации окончательно решенным, что обусловлено рядом факторов. Одним из способов повышения точности прогнозирования сердечно-сосудистого риска может стать дополнение традиционных шкал сердечно-сосудистыми биомаркерами, которые обладают определенными свойствами. Сердечные тропонины являются самыми надежными кардиоваскулярными биомаркерами в области диагностики острого инфаркта миокарда. Однако, как показали исследования последних лет, у них есть потенциал использования в качестве дополнения к шкалам стратификации сердечно-сосудистого риска. Они соответствуют требованиям, предъявляемым к новым биомаркерам: кардиоспецифичность, прогностическая ценность, изменение уровней при определенных воздействиях, направленных на снижение риска, доказательная ценность, экономическая эффективность. Большое количество крупных международных исследований, проведенных на популяционном уровне, демонстрируют ассоциацию повышенного уровня сердечного тропонина I с развитием неблагоприятных сердечно-сосудистых событий. Для оценки целесообразности дополнения традиционных шкал риска новыми биомаркерами, включая тропонин I, в Российской Федерации стартует исследование по оценке значимости ряда биомаркеров в прогнозировании риска сердечно-сосудистых исходов у мужчин и женщин трудоспособного возраста.</p></abstract><trans-abstract xml:lang="en"><p>Risk stratification is an important aspect of effective control of cardiovascular diseases. Risk scores based on conventional risk factors did not solve the stratification problem completely due to a number of factors. One way to increase the accuracy of predicting cardiovascular risk may be to supplement conventional scores with cardiovascular biomarkers. Cardiac troponins are the most reliable cardiovascular biomarkers in the diagnosis of acute myocardial infarction. However, recent studies have shown that they have the potential to use as an addition to cardiovascular risk stratification scores. Cardiac troponins meet the requirements for new biomarkers: cardiospecificity, prognostic value, change in levels in response to risk reduction actions, evidence value, economic efficiency. A large number of population studies demonstrate the association of elevated levels of cardiac troponin I with adverse cardiovascular events.
