Основными причинами низкой частоты интервенций служит сложность процедуры и риск интраоперационных осложнений. В связи с этим особую роль играет поиск предикторов безуспешного эндоваскулярного вмешательства при ХОКА. Это позволит производить тщательный отбор пациентов, у которых можно наблюдать максимальный эффект от операции.</p><sec><title>Цель</title><p>Цель. Выявить предикторы безуспешной эндоваскулярной реканализации ХОКА.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Настоящая работа представляет собой ретроспективное исследование, в которое были включены в период с ноября 2017 по июнь 2019гг пациенты (n=180) с хронической формой ишемической болезни сердца, у которых имелось многососудистое поражение в сочетании с ХОКА. У всех пациентов предпринималась попытка достижения полной реваскуляризации миокарда. В зависимости от успеха вмешательства пациенты были разделены на две группы: полной реваскуляризации (восстановление перфузии) миокарда и неполной реваскуляризации миокарда. Период наблюдения составил 12 мес.</p></sec><sec><title>Результаты</title><p>Результаты. Все исходные характеристики пациентов в сравниваемых группах были схожими. Частота успешной реканализации (восстановление тока крови по коронарной артерии) окклюзии составила 79,5%. Многофакторный регрессионный анализ показал, что кальциноз ХОКА (p&lt;0,001), исходный SYNTAX Score (Synergy between Percutaneous Coronary Intervention with TAXUS and Cardiac Surgery) &gt;32 баллов (р=0,006), длина ХОКА &gt;30 мм (р=0,046) и локализация ХОКА в огибающей артерии (р&lt;0,01) являются статистически значимыми предикторами безуспешной эндоваскулярной реканализации ХОКА. Для оценки прогностической значимости модели проведен ROC-анализ, и рассчитана площадь под кривой AUC (Area Under Curve), значение которой составило 0,87, что говорит о высоком предикативном качестве модели. Чувствительность и специфичность модели составили 78 и 81%, соответственно.</p></sec><sec><title>Заключение</title><p>Заключение. Исследование показало, что значимыми предикторами безуспешной реканализации ХОКА служат наличие кальциноза ХОКА, SYNTAX Score &gt;32 баллов, длина ХОКА &gt;30 мм и локализация ХОКА в огибающей артерии.</p></sec></abstract><trans-abstract xml:lang="en"><p>The prevalence of endovascular interventions for coronary chronic total occlusion (CTO) remains small worldwide. This is due to the complexity of procedure and the risk of intraoperative complications. In this regard, the search for predictors of unsuccessful endovascular intervention in CTO plays a special role. This will allow for a careful selection of patients with the most favorable expectation effect of the operation. Aim. To identify predictors of unsuccessful endovascular recanalization of CTO.</p><sec><title>Material and methods</title><p>Material and methods. This retrospective study included 180 patients with chronic coronary artery disease (CAD) in the period from November 2017 to June 2019, who had multivessel lesion in combination with CTO. In all patients, an attempt was made to achieve complete myocardial revascularization. Depending on the success of procedure, the patients were divided into two groups: complete and incomplete myocardial revascularization. The follow-up period was 12 months.</p></sec><sec><title>Results</title><p>Results. All baseline characteristics of patients in the compared groups were similar. The successful recanalization rate of occlusion was 79,5%. Multivariate regression analysis showed that calcified CTO (p&lt;0,001), baseline SYNTAX (Synergy between Percutaneous Coronary Intervention with TAXUS and Cardiac Surgery) score &gt;32 (p=0,006), CTO length &gt;30 mm (p=0,046) and CTO of circumflex artery (p&lt;0,01) are significant predictors of unsuccessful endovascular recanalization of CTO. To assess the predictive value of the model, a ROC analysis was carried out, and the area under the curve (AUC) was calculated. The AUC was 0,87, which indicates a high predictive quality of the model. The sensitivity and specificity of the model were 78 and 81%, respectively.</p></sec><sec><title>Conclusion</title><p>Conclusion.
