Оценка взаимосвязи сахарного диабета 2 типа (СД2) с течением и исходами COVID-19 (COronaVIrus Disease 2019, новая коронавирусная инфекция 2019) в зависимости от клинико-лабораторных показателей и сопутствующих заболеваний.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведен ретроспективный анализ клинических исходов у 1985 пациентов с подтвержденным диагнозом COVID-19, которые были госпитализированы в городскую клиническую больницу имени В. П. Демихова в период с 23.03.2020 по 01.06.2020гг. Впоследствии 232 пациента были исключены из исследования из-за их перевода в другой стационар. В финальный анализ включены 1753 пациента, из которых 311 (17,7%) пациентов имели СД2.</p></sec><sec><title>Результаты</title><p>Результаты. СД2 был ассоциирован с неблагоприятным исходом COVID-19: летальность без СД2 — 7,4%, с СД2 — 16,1% (p&lt;0,001). Возраст &gt;60 лет увеличивал летальность пациентов с COVID-19, особенно при сопутствующем СД2: пациенты до 60 лет без СД2 — 2,2%, с СД2 — 5,4% (р&lt;0,05); пациенты &gt;60 лет без СД2 — 15,6%, с СД2 — 22,1% (р&lt;0,01). Пол не был ассоциирован с летальностью среди пациентов с COVID-19. Среди пациентов без СД2 кривая летальности в зависимости от уровня гликемии носила U-образный характер: 3,7% при гликемии &lt;6,1 ммоль/л, 1,1% при гликемии в пределах 6,1-7,0 ммоль/л, 10,8% при гликемии &gt;7,0 ммоль/л (p&lt;0,001). У пациентов с COVID-19 и СД2 выявлена прямая зависимость между уровнем гликемии и летальностью: при гликемии &lt;6,1 ммоль/л летальность отмечалась в 8,3% случаев, при гликемии в пределах 6,1-7,0 ммоль/л — в 11,8% случаев, при гликемии &gt;7 ммоль/л — в 15,6% случаев (p&lt;0,001). Любое сопутствующее заболевание ухудшало исход у пациентов с COVID-19. Самый высокий процент летальности отмечался при наличии хронической обструктивной болезни легких — 46,2% (p&lt;0,001), ишемической болезни сердца — 26,3% (p&lt;0,001), злокачественных образований — 25% (p&lt;0,001). Статистически значимо СД2 увеличивал летальность среди пациентов с хронической обструктивной болезнью легких — 33,3 vs 75% (р&lt;0,05), бронхиальной астмой — 7,1 vs 60% (р&lt;0,001) и злокачественных образований — 12,5 vs 62,5% (р&lt;0,01).</p></sec><sec><title>Заключение</title><p>Заключение. СД2 ассоциирован с неблагоприятным исходом COVID-19, особенно среди пациентов &gt;60 лет и высокими показателями гликемии. Любое сопутствующее заболевание ухудшало исход заболевания. Основной причиной смерти от COVID-19 являлось развитие осложнений: острого респираторного дистресс-синдрома, сепсиса, острой почечной недостаточности, тромботических осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the relationship between type 2 diabetes (T2D) and the course and outcomes of coronavirus disease 2019 (COVID-19), depending on clinical and laboratory parameters and comorbidities.</p></sec><sec><title>Material and methods</title><p>Material and methods. A retrospective analysis of clinical outcomes was carried out in 1985 patients with documented COVID-19 who were admitted to the V. P. Demikhov City Clinical Hospital in the period from March 23, 2020 to June 01, 2020. Subsequently, 232 patients were excluded due to their transfer to another hospital. The final analysis included 1753 patients, of which 311 (17,7%) patients had T2D.</p></sec><sec><title>Results</title><p>Results. T2D was associated with an unfavorable outcome of COVID-19: mortality with and without — 16,1% and 7,4% (p&lt;0,001). Age &gt;60 years increased the mortality of patients with COVID-19, especially with concomitant T2D: patients &lt;60 years of age without T2D — 2,2%, with T2D — 5,4% (p&lt;0,05); patients &gt;60 years of age without T2D — 15,6%, with T2D — 22,1% (p&lt;0,01). Sex was not associated with mortality among COVID-19 patients. Among patients without T2D, the glycemia-depended mortality curves were U-shaped: 3,7% with glycemia &lt;6,1 mmol/L, 1,1% with glycemia in the range of 6,17,0 mmol/L, 10,8% with glycemia &gt;7,0 mmol/L (p&lt;0,001). In patients with COVID-19 and T2D, a direct relationship was found between the level of glycemia and mortality: with glycemia &lt;6,1 mmol/L, mortality rate was 8,3%, while with glycemia in the range of 6,1-7,0 mmol/L — 11,8% and glycemia &gt;7 mmol/L — 15,6% (p&lt;0,001). Any comorbidity worsened the outcome in COVID-19 patients. The highest mortality rate was observed in the presence of chronic obstructive pulmonary disease (46,2%; p&lt;0,001), coronary artery disease (26,3%; p&lt;0,001), cancer 25% (p&lt;0,001). T2D significantly increased mortality among patients with chronic obstructive pulmonary disease — 33,3 vs 75% (p&lt;0,05), asthma — 7,1 vs 60% (p&lt;0,001), and cancer — 12,5 vs 62,5% (p&lt;0,01).</p></sec><sec><title>Conclusion</title><p>Conclusion. T2D is associated with unfavorable COVID-19 outcomes, especially among patients &gt;60 years of age and high blood glucose levels. Any concomitant disease worsened the outcome of the disease.
