Изучить возможность использования качественных градаций риска инсульта для оценки вероятности выявления у  больных с персистирующей фибрилляцией предсердий (ФП) тромба в ушке левого предсердия (УЛП) и решения вопроса о целесообразности проведения чреспищеводной эхокардиографии (ЧП-ЭхоКГ) перед восстановлением синусового ритма.</p></sec><sec><title>Материал и  методы</title><p>Материал и  методы. ЧП-ЭхоКГ перед плановой кардиоверсией выполнена 590 больным персистирующей ФП, из которых у  316 (53,6%) риск инсульта был высоким (клапанная ФП, больные гипертрофической кардиомиопатией, оценка по шкале CHA2DS2-VASc &gt;1 у мужчин и &gt;2 у женщин), а у 274 (46,4%) — невысоким. Адекватную антикоагулянтную терапию (АКТ) ≥3 нед. до планируемой кардиоверсии получали 164 (51,9%) пациента с высоким риском инсульта и  151 (55,1%) пациент с  невысоким риском. Остальные пациенты либо не получали адекватной АКТ, либо получали ее &lt;3 нед.</p></sec><sec><title>Результаты</title><p>Результаты. В  группе больных, получавших адекватную АКТ ≥3 нед., тромб в  УЛП был выявлен у  23 (14,0%) пациентов с  высоким риском инсульта и  у  8 (5,3%) пациентов с  невысоким риском (p&lt;0,05). У  больных, не получавших адекватную АКТ или получавших ее &lt;3 нед., тромб в  УЛП был выявлен у  60 (39,5%) пациентов с высоким риском инсульта и у 22 (17,9%) пациентов с невысоким риском (p&lt;0,005). Таким образом, высокий риск инсульта почти в 3 раза повышает шансы выявления тромба в  УЛП как у  больных, не получающих адекватную АКТ (отношение шансов (ОШ) =2,99, 95% доверительный интервал: 1,70-5,26; р&lt;0,001), так и у больных, получающих адекватную АКТ (ОШ =2,92, 95% доверительный интервал: 1,26-6,74; р=0,012).</p></sec><sec><title>Заключение</title><p>Заключение. У больных с персистирующей ФП с невысоким риском инсульта по шкале CHA2DS2-VASc целесообразно использовать тактику восстановления синусового ритма после выполнения ЧПЭхоКГ без предварительной 3-недельной АКТ. У больных с высоким риском инсульта ЧП-ЭхоКГ перед кардиоверсией целесообразно выполнять даже после адекватной антикоагулянтной подготовки.</p></sec><sec><title> </title><p> </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the potential of stroke risk assessment to evaluate the probability of detecting left atrial appendage (LAA) thrombus in patients with persistent atrial fibrillation (AF) to consider performing transesophageal echocardiography (TEE) before cardioversion.</p></sec><sec><title>Material and methods</title><p>Material and methods. TEE before elective cardioversion was performed in 590 patients with persistent AF, of whom 316 (53,6%) had a high stroke risk (valvular AF, hypertrophic cardiomyopathy, CHA2DS2-VASc score &gt;1 in men and &gt;2 in women), and 274 (46,4%) — not high. Adequate anticoagulation at least 3 weeks prior to elective cardioversion was received by 164 (51,9%) patients with a high stroke risk and 151 (55,1%) patients with a low risk. The rest of patients either did not receive adequate anticoagulation or received it for less than 3 weeks.</p></sec><sec><title>Results</title><p>Results. In the group of patients who received anticoagulation at least 3 weeks, LAA thrombus was detected in 23 (14,0%) patients with a high stroke risk and in 8 (5,3%) patients with a low risk (p&lt;0,05). In patients who did not receive adequate anticoagulation or received it for less than 3 weeks, LAA thrombus was identified in 60 (39,5%) patients with a high stroke risk and in 22 (17,9%) patients with a low risk (p&lt;0,005). Thus, a  high stroke risk almost 3 times increases the likelihood of LAA thrombus detection in patients who did not receive adequate anticoagulation (odds ratio, 2,99; 95% confidence interval: 1,70-5,26;p&lt;0,001) and in patients receiving adequate anticoagulation (odds ratio, 2,92; 95% confidence interval: 1,26-6,74; p=0,012).</p></sec><sec><title>Conclusion</title><p>Conclusion. In patients with persistent AF with a low stroke risk according to CHA2DS2-VASc score, TEE before sinus rhythm restoration in patients without 3-week anticoagulation should be considered.
