Создание усовершенствованного алгоритма оценки вероятности развития сердечно-сосудистых осложнений (ССО) у больных низкого/умеренного риска с использованием комплекса “классических” и “новых” факторов.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В работу включено 700 больных с риском по SCORE (Systematic Coronary Risk Evaluation) &lt;5%, обследованных в 2009-2010гг. Помимо стандартного обследования проводились биохимический анализ крови с определением высокочувствительного С-реактивного белка (вчСРБ), сфигмография. В 2019г проведен телефонный обзвон участников для выявления ССО за прошедшее время: смерть от сердечно-сосудистых заболеваний, инфаркты миокарда, нестабильная стенокардия, нарушения мозгового кровообращения, реваскуляризация. Отклик — 79,6% (557 человек: 100 мужчин и 457 женщин).</p></sec><sec><title>Результаты</title><p>Результаты. ССО были у 48 (8,6%) пациентов. Риск ССО повышает уровень систолического артериального давления (САД) &gt;130 мм рт.ст. — отношение шансов (ОШ) 1,9 (95% доверительный интервал (ДИ) 1,0-3,6), вчСРБ &gt;2,3 мг/л — ОШ 2,2 (95% ДИ: 1,5-4,1), мочевины &gt;8 ммоль/л — ОШ 5,1 (95% ДИ: 1,2-21,0), кардио-лодыжечный сосудистый индекс (CAVI) &gt;8,05 — ОШ 1,25 (95% ДИ: 1,0-1,6). У пациентов с сочетанием нарушения ≥2 параметров липидного спектра, САД &gt;130 мм рт.ст., вчСРБ &gt;2,3 мг/л и скорость пульсовой волны &gt;13 м/с вероятность развития ССО (включая смерть по сердечно-сосудистым причинам) увеличивается в 3,55 раз (95% ДИ: 1,32-7,67).</p></sec><sec><title>Заключение</title><p>Заключение. Скорость пульсовой волны, CAVI, уровни мочевины, вчСРБ могут рассматриваться как дополнительные факторы риска развития ССО у больных низкого/умеренного риска, рассчитанного по стандартным шкалам. Сочетания “классических” и “новых” факторов риска демонстрирует кумулятивный эффект.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To create an advanced algorithm for predicting cardiovascular events (CVE) in low/moderate risk patients using a complex of traditional and new factors.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 700 patients with Systematic Coronary Risk Evaluation (SCORE) &lt;5%, examined in 20092010. In addition to standard investigations, blood biochemistry tests, including high-sensitivity C-reactive protein (hsCRP), and sphygmography were carried. In 2019, a follow-up phone call was made to participants to identify recent CVEs: cardiovascular death, myocardial infarction, unstable angina, stroke, revascularization. The response rate was 79,6% (n=557; men, 100; women, 457).</p></sec><sec><title>Results</title><p>Results. CVEs were observed in 48 (8,6%) patients. The risk of CVEs increases systolic blood pressure (SBP) &gt;130 mmHg (odds ratio (OR), 1,9 (95% confidence interval (CI), 1,0-3,6)), hsCRP &gt;2,3 mg/L (OR, cardio-ankle vascular index (CAVI) &gt;8,05 (OR, 1,25 (95% CI, 1,0-1,6)). In patients with a combination of ≥2 lipid profile abnormalities, SBP &gt;130 mm Hg, hsCRP &gt;2,3 mg/L and pulse wave velocity &gt;13 m/s, the probability of developing CVEs (including cardiovascular death) increases 3,55 times (95% CI, 1,32-7,67).</p></sec><sec><title>Conclusion</title><p>Conclusion. Levels of pulse wave velocity, CAVI, urea and hsCRP should be considered as additional risk factors for CVE in patients with low/moderate risk, estimated using standard scales.
