Провести сравнительный анализ качества жизни (КЖ) пациентов с ишемической болезнью сердца (ИБС) в сочетании с длительно персистирующей формой фибрилляции предсердий (ФП) в отдаленном послеоперационном периоде в зависимости от выбранной тактики хирургического лечения сочетанной патологии.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведен анализ изменения КЖ в отдаленном послеоперационном периоде (12 и 24 мес.) у 116 пациентов с ИБС и сопутствующей длительно персистирующей формой ФП, которым избирательно была проведена биатриальная аблация (БА) или изолированная левопредсердная аблация (ЛПА) с одномоментным аортокоронарным шунтированием в условиях искусственного кровообращения. Для оценки КЖ использовали неспецифический опросник SF-36 (Medical Outcomes Study 36-Item Form Health Status Survey). Анкетирование пациентов проводили на дооперационном этапе и в отдаленном послеоперационном периоде (12 и 24 мес.).</p></sec><sec><title>Результаты</title><p>Результаты. Показатели всех шкал SF-36 существенно улучшаются после открытого хирургического лечения сочетанного поражения в отдаленном послеоперационном периоде (24 мес.) при обеих стратегиях лечения (БА и ЛПА) ФП. В группе БА через 12 мес. 74% пациентов не ощущали аритмию, и лишь 38,5% пациентов в группе ЛПА относились к классу 1 шкалы EHRA (European Heart Rhythm Association, Европейская ассоциация аритмологов) (р=0,001). Через 24 мес. при общем сравнении отмечалась статистически значимая разница между двумя группами по симптоматике аритмии (р=0,014), с сохранением преимущества группы БА. Через 12 и 24 мес. ни у одного пациента обеих сравниваемых групп не было тяжелой стенокардии IV функционального класса.</p></sec><sec><title>Заключение</title><p>Заключение. Показано улучшение показателей шкалы SF-36 через 24 мес. после хирургического лечения ИБС и длительно персистирующей формы ФП вне зависимости от стратегии применения аблации. Устранение симптомов стенокардии и длительное сохранение синусового ритма позволяют улучшить КЖ пациентов в отдаленном послеоперационном периоде.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To analyze quality of life (QOL) of patients with coronary artery disease (CAD) in combination with long-standing persistent atrial fibrillation (AF) in the long-term postoperative period, depending on chosen surgical strategy for concomitant pathology.</p></sec><sec><title>Material and methods</title><p>Material and methods. The analysis of QOL changes in the long-term postoperative period (12 and 24 months) in 116 patients with CAD and concomitant long-term persistent AF who selectively underwent biatrial (BA) or isolated left atrial (LA) ablation with simultaneous on-pump coronary artery bypass grafting. To assess QOL, a non-specific Medical Outcomes Study 36-Item Form Health Status Survey (SF-36) questionnaire was used. Patients were questioned in preoperative and long-term postoperative periods (12 and 24 months).</p></sec><sec><title>Results</title><p>Results. All SF-36 parameters significantly improve after open surgical treatment in the long-term postoperative period (24 months) with both treatment strategies (BA and LA ablation) for AF. In the BA ablation group, 74% of patients did not have arrhythmia after 12 months, and only 38,5% of patients in the LA ablation group belonged to European Heart Rhythm Association (EHRA) score class 1 (p=0,001). After 24 months, a comparison revealed a significant diff erence between the two groups in arrhythmia symptoms (p=0,014), with maintaining the advantage of the BA ablation group. After 12 and 24 months, none of the patients in both compared groups had severe class IV angina.</p></sec><sec><title>Conclusion</title><p>Conclusion. SF-36 parameters were improved 24 months after surgical treatment of CAD and long-standing persistent AF, regardless of the ablation strategy.
