Оценить распределение сердечного тропонина I (cTnI) в выборке одного из регионов-участников исследования ЭССЕ-РФ (Эпидемиология сердечно-сосудистых заболеваний в различных регионах Российской Федерации) и изучить его ассоциации с факторами риска (ФР) и европейской шкалой риска SCORE (Systematic Coronary Risk Evaluation).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Материалом послужила представительная выборка из населения 25-64 лет Вологодского региона (n=1591). Дизайн исследования: наблюдательное одномоментное. В анализ включали пол, возраст, поведенческие и кардиометаболические ФР и биомаркеры, сердечно-сосудистые заболевания (ССЗ) в анамнезе, SCORE. Уровень статистической значимости между возрастно-половыми группами составил p&lt;0,05.</p></sec><sec><title>Результаты</title><p>Результаты. Впервые было построено распределение уровня сTnI на выборке российского населения одного из регионов-участников исследования ЭССЕ-РФ, выявлена его асимметричность. Медиана уровня cTnI в выборке составила 1,5 пг/мл (95% доверительный интервал 0,80-2,50). Обращает на себя внимание высокий уровень 99-го процентиля cTnI у мужчин в возрасте 45-54 лет (55,3 пг/мл); при этом 99-й процентиль по всей выборке у мужчин составил 47,7 пг/мл, у женщин — 13,3 пг/мл. Log уровня cTnI статистически значимо увеличивается с возрастом как у мужчин (p&lt;0,0001), так и у женщин (p&lt;0,0001), но у мужчин быстрее. Отмечен более высокий уровень cTnI у мужчин в молодом возрасте сравнительно с уровнем cTnI у их сверстниц. В многофакторном анализе получены значимые ассоциации уровня cTnI с полом, возрастом, содержанием в крови холестерина липопротеинов низкой плотности, абдоминальным ожирением, артериальной гипертонией и концентрацией мозгового натрийуретического пептида. Выявлена недостаточная точность классификации участников исследования по риску SCORE.</p></sec><sec><title>Заключение</title><p>Заключение. Распределение cTnI в выборке российского региона из исследования ЭССЕ-РФ резко сдвинуто влево, медиана составила 1,5 пг/мл. Выявлены более высокие уровни cTnI у молодых мужчин по сравнению со сверстницами. Найдены значимые связи cТnI с ожирением (индексом массы тела, артериальной гипертонией, повышенным холестерином липопротеинов низкой плотности и концентрацией мозгового натрийуретического пептида). Установлено, что SCORE недостаточно точно классифицирует лиц с высоким и умеренным уровнем cТnI.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To assess the distribution of cardiac troponin I (cTnI) in a sample of the region from the ESSE-RF study and to study its associations with risk factors (RFs) and the Systematic Coronary Risk Evaluation (SCORE) system.</p></sec><sec><title>Material and methods</title><p>Material and methods. This observational cross-sectional study includes a representative sample of the population aged 25-64 years of the Vologda region (n=1591). The analysis included sex, age, behavioral and cardiometabolic RFs, biomarkers, prior cardiovascular diseases (CVDs), and SCORE. The differences between age-sex groups were considered significant at p&lt;0,05.</p></sec><sec><title>Results</title><p>Results. According to study results, asymmetry in cTnI distribution among population was revealed. The median cTnI level in the sample was 1,5 pg/ml (95% confidence interval, 0,80-2,50). Noteworthy is the high level of the 99th percentile of cTnI in men aged 45-54 years (55,3 pg/ml). At the same time, the 99th percentile for the entire sample in men was 47,7 pg/ml, while in women — 13,3 pg/ml. The cTnI level log increases significantly with age in both men (p&lt;0,0001) and women (p&lt;0,0001), but faster in men. There was a higher level of cTnI in young men compared to same-age women. In multivariate analysis, significant associations of cTnI levels with sex, age, blood low-density lipoprotein cholesterol, abdominal obesity, hypertension, and brain natriuretic peptide level were obtained. Insufficient accuracy of classification of study participants by SCORE risk was revealed.</p></sec><sec><title>Conclusion</title><p>Conclusion. The distribution of cTnI in the sample of the Russian region from the ESSE-RF study is sharply shifted to the left (median, 1,5 pg/ml). Higher cTnI levels were revealed in young men compared to their female peers. Significant associations of cTnI with obesity (body mass index, hypertension, elevated low-density lipoprotein cholesterol, and brain natriuretic peptide concentration) were found.
