В настоящем обзоре подробно обсуждается физиологическое значение сигнального пути, ассоциированного с растворимой гуанилатциклазой, изложены причины снижения ее активности при сердечной недостаточности и возможные последствия в виде нарушения функции сердца и сосудов и прогрессирования заболевания. Рассматриваются фармакологические методы стимуляции образования циклического гуанозинмонофосфата при помощи препаратов с разными механизмами действия: приведены данные клинических исследований относительно их эффективности и безопасности. Перспективным подходом можно считать стимуляцию растворимой гуанилатциклазы, сопровождавшуюся благоприятными эффектами в доклинических исследованиях, а также в недавно завершившемся исследовании III фазы VICTORIA.</p></abstract><trans-abstract xml:lang="en"><p>Heart failure is a severe disease with an unfavorable prognosis, which requires intensification of therapy and the search for novel approaches to treatment. In this review, the physiological significance of soluble guanylate cyclase-related signaling pathway, reasons for decrease in its activity in heart failure and possible consequences are discussed. Pharmacological methods of stimulating the production of cyclic guanosine monophosphate using drugs with different mechanisms of action are considered. Data from clinical studies regarding their effectiveness and safety are presented.
