Провести сравнительный анализ влияния фармакотерапии блокаторами рецепторов ангиотензина II (азилсартан, телмисартан) на динамику маркеров фиброза и иммунного воспаления у больных гипертонической болезнью (ГБ) на фоне хронической болезни почек (ХБП) после перенесенного ишемического инсульта (ИИ).</p></sec><sec><title>Материал и  методы</title><p>Материал и  методы. В  исследование включено 76 больных ГБ в  возрасте 60-74 лет (средний возраст 66±5 лет), имеющих ХБП, перенесших ИИ. Пациенты рандомизированы случайным образом на группы фармакотерапии: 38 больных в группу телмисартана и 36 больных — азилсартана. Контрольную группу составили 20 человек (средний возраст 63±2 года) с ГБ, без ХБП и ИИ в анамнезе. Уровни матриксной металлопротеиназы 9 типа (ММП-9) и тканевого ингибитора матриксных металлопротеиназ 1 типа (ТИМП-1) определяли методом иммуноферментного анализа (ELISA Kit, США); уровень интерлейкина-1β (ИЛ-1β), фактора некроза опухоли α (ФНО-α), интерферона γ (ИНФ-γ), моноцитарного хемоаттрактантного протеина 1 (МСР-1) — ЗАО “Вектор-Бест”, Россия.</p></sec><sec><title>Результаты</title><p>Результаты. Применение азилсартана в  течение 6 мес. привело к  снижению уровня ММП-9 на 19,9% (р&lt;0,01), ТИМП-1  — на 7,5% (р&lt;0,05); ИЛ-1β  — на 7,8%, ФНО-α  — на 13,5%, ИНФ-γ  — на 7,1%, МСР-1 — на 13% (р&lt;0,05). При применении телмисартана уровень ММП-9 снизился на 39,1% (р&lt;0,01), ТИМП-1  — на 16,4%, ИЛ-1β  — на 10,1% (р&lt;0,05), ФНО-α — на 20,8% (р&lt;0,01), ИНФ-γ — на 14,6% (р&lt;0,05), МСР-1  — на 21,3% (р&lt;0,01). При межгрупповом сравнении установлено, что при применении телмисартана динамика снижения уровня ММП-9 была на 19,2% (р&lt;0,01), ТИМП-1 на 7,2% (р&lt;0,05), ФНО-α на 7,3% (р&lt;0,05), ИНФ-γ на 7,5% (р&lt;0,05), МСР-1 на 8,3% (р&lt;0,05) более выражена, чем при применении азилсартана. При применении телмисартана СКФ увеличивалась более значимо (на 14,2%, р&lt;0,05) по сравнению с азилсартаном.</p></sec><sec><title>Заключение</title><p>Заключение. Фармакотерапия телмисартаном у  больных ГБ на фоне ХБП после перенесенного инсульта через 6 мес. сопровождалась более выраженным снижением маркеров фиброза миокарда (ММП-9, ТИМП-1), иммунного воспаления (ФНО-α, ИНФ-γ, МСР-1) и улучшением функции почек по сравнению с азилсартаном.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To compare the effect of angiotensin II receptor blocker therapy (azilsartan, telmisartan) on fibrosis and immune inflammation markers in hypertensive patients with chronic kidney disease (CKD) after ischemic stroke (IS).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 76 hypertensive patients aged 60-74 years (mean age, 66±5 years) with CKD after IS. Patients were randomly divided into following pharmacotherapy groups: 38 patients — telmisartan group; 36 patients — azilsartan group. The control group consisted of 20 hypertensive people (mean age, 63±2 years) without a history of CKD and IS. The levels of matrix metalloproteinase 9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) were determined by enzyme-linked immunosorbent assay (ELISA Kit, USA). The levels of interleukin-1β (IL-1β), tumor necrosis factor α (TNF-α), interferon γ (INF-γ), monocytic chemoattractant protein 1 (MCP-1) were assessed using Vector-Best kit (Russia).</p></sec><sec><title>Results</title><p>Results. Six-month azilsartan therapy led to a decrease in the levels of MMP-9 by 19,9% (p&lt;0,01), TIMP-1 by 7,5% (p&lt;0,05), IL-1β by 7,8%, TNF-α by 13,5%, INF-γ by 7,1%, MCP-1 by 13% (p&lt;0,05). Telmisartan therapy was associated with a decrease in the levels of MMP-9 by 39,1% (p&lt;0,01), TIMP-1 by 16,4%, IL-1β by 10,1% (p&lt;0,05), TNF-α by 20,8% (p&lt;0,01), INF-γ by 14,6% (p&lt;0,05), MCP-1 by 21,3% (p&lt;0,01). Intergroup comparison revealed more pronounced changes in the levels of MMP-9 by 19,2% (p&lt;0,01), TIMP-1 by 7,2% (p&lt;0,05), TNF-α by 7,3% (p&lt;0,05), INF-γ by 7,5% (p&lt;0,05), and MCP-1 by 8,3% (p&lt;0,05) when using telmisartan compared to azilsartan. When using telmisartan, the increase in glomerular filtration rate (GFR) was 14,2% (p&lt;0,05) higher compared to azilsartan.</p></sec><sec><title>Conclusion</title><p>Conclusion.
