Сравнительный анализ влияния фармакотерапии бета-адреноблокаторами (бисопролол и небиволол) на динамику маркеров фиброза сосудистой стенки и эндотелиальную функцию у пожилых пациентов с гипертонической болезнью (ГБ) после перенесенного ишемического инсульта (ИИ).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведено проспективное когортное исследование, в которое были включены 75 больных с ГБ, поступившие в стационар в остром периоде ИИ. Средний возраст больных — 67±6 лет. Средний балл по NIHSS (National Institutes of Health Stroke Scale) — 7±3. Длительность наблюдения — 6 мес. Контрольная группа — 20 человек пожилого возраста с ГБ без ИМ в анамнезе. Пациенты разделены на группы по признаку медикаментозной терапии: первая (n=38) получала бисопролол; вторая (n=37) — небиволол. Уровень матриксной металлопротеиназы-9 (ММП-9) и тканевого ингибитора матриксных металлопротеиназ-1 (ТИМП-1) определяли методом иммуноферментного анализа (ELISAKit, США). Ультразвуковое (УЗ) исследование сосудов проводили на УЗ-сканере LOGIQP9 (GE) по методу Celermajer.</p></sec><sec><title>Результаты</title><p>Результаты. При применении небиволола в течение 6 мес. выявлено снижение уровня ММП-9 на 30,2% (р&lt;0,01), ТИМП-1 — на 15,6% (р&lt;0,05); при приеме бисопролола уровень ММП-9 снизился на 14,5% (р&lt;0,05), ТИМП-1 — не изменился. При межгрупповом сравнении установлено, что при применении небиволола снижение уровня ММП-9 на 15,7% (р&lt;0,05), ТИМП-1 на 9,7% (р&lt;0,05), ММП-9/ТИМП-1 на 7,8% (р&lt;0,05) выше чем на фоне бисопролола. Через 6 мес. фармакотерапии бисопрололом отмечено снижение доли больных, имеющих значительную степень дисфункции эндотелия (ДЭ) на 7,9% (р&lt;0,05). Двое больных из группы принимавших неби-волол, перешли в категорию незначительной ДЭ, количество больных с умеренной ДЭ увеличилось на 19% (р&lt;0,01), значительной ДЭ — уменьшилось на 24,4% (р&lt;0,01).</p></sec><sec><title>Заключение</title><p>Заключение. Полученные результаты свидетельствуют, что бе-та-адреноблокатор небиволол в средней дозе 8,55±1,75 мг/сут. достоверно снижает фиброз сосудистой стенки, нормализует соотношение маркеров синтеза и деградации коллагена, улучшает вазодилатирующие свойства эндотелия плечевой артерии по сравнению с бисопрололом у пожилых больных ГБ, перенесших ИИ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To compare the effect of beta-blocker therapy (bisoprolol and nebivolol) on the dynamics of fibrotic and vascular endothelial dysfunction markers in elderly hypertensive patients after ischemic stroke (IS).</p></sec><sec><title>Material and methods</title><p>Material and methods. This prospective cohort study included 75 hypertensive patients who were admitted to the hospital due to IS. The mean age of patients was 67±6 years. The average National Institutes of Health Stroke Scale (NIHSS) score was 7±3. The followup period was 6 months. The control group consisted of 20 elderly people with hypertension without prior myocardial infarction. The patients were divided into groups based on received therapy: group 1 (n=38) — bisoprolol; group 2 (n=37) — nebivolol. The level of matrix metalloproteinase-9 (MMP-9) and tissue inhibitor of metalloproteinase-1 (TIMP-1) was determined by enzyme-linked immunosorbent assay (ELISAKit, USA). Vascular ultrasound was carried out using a LOGIQP9 (GE) system according to the Celermajer method.</p></sec><sec><title>Results</title><p>Results. After 6-month nebivolol, we revealed a decrease in the level of MMP-9 by 30,2% (p&lt;0,01), TIMP-1 by 15,6% (p&lt;0,05). After 6-month bisoprolol therapy, the level of MMP-9 decreased by 14,5% (p&lt;0,05), while TIMP-1 did not change. Intergroup comparison found that when using nebivolol, there was a higher decrease in the level of MMP-9 by 15,7% (p&lt;0,05), TIMP-1 by 9,7% (p&lt;0,05), MMP-9/TIMP-1 by 7,8% (p&lt;0,05) than with bisoprolol therapy. After 6-month bisoprolol therapy, there was a decrease in the proportion of patients with severe endothelial dysfunction (ED) by 7,9% (p&lt;0,05). Two patients from the nebivolol group moved into mild ED category. The number of patients with moderate ED increased by 19% (p&lt;0,01), while prevalence of severe ED decreased by 24,4% (p&lt;0,01).</p></sec><sec><title>Conclusion</title><p>Conclusion.
