Выявить и охарактеризовать ассоциации между наличием и выраженностью атеросклероза различной локализации с уровнем биохимических показателей крови и оценить возможность их использования в качестве маркеров метаболических нарушений, обусловливающих повышенный атерогенный потенциал.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включено 216 пациентов (53% мужчин) в возрасте 24-87 лет (средний возраст 61,5±10,73 лет). Всем пациентам проведена коронароангиография, дуплексное сканирование сонных (СА) и бедренных артерий (БА) с диагностикой наличия и выраженности атеросклероза коронарных артерий (КА), СА и БА. В сыворотке/плазме крови выполнен анализ биохимических показателей стандартными методами.</p></sec><sec><title>Результаты</title><p>Результаты. На основании анализа профиля циркулирующих биохимических маркеров сформированы диагностические комплексы, позволяющие детектировать наличие и выраженность атеросклероза разной локализации. Согласно полученным данным, детерминантами наличия атеросклероза КА и СА являются дисфункция сосудистого эндотелия (концентрация метаболитов оксида азота &lt;36,0 мкмоль/л) и повышенный уровень креатинина (≥73,0 мкмоль/л). Специфическими маркерами, связанными с выраженным атеросклерозом КА и БА (но не СА) оказались низкий уровень холестерина липопротеинов высокой плотности (≤1,0/1,2 ммоль/л мужчины/женщины) и повышенный уровень С-реактивного белка (≥1,0 мг/л). Выраженный периферический атеросклероз (поражение СА и БА) ассоциировался с гипергликемией (глюкоза ≥6,1 ммоль/л), а выраженный атеросклероз БА — с гиперинсулинемией (инсулин ≥14,0 мкЕд/мл).</p></sec><sec><title>Заключение</title><p>Заключение. Анализ ассоциаций циркулирующих биомаркеров с локализацией и выраженностью атеросклероза позволил выявить ряд маркеров метаболических нарушений, обусловливающих повышенный атерогенный потенциал. Выделены универсальные параметры, связанные с атеросклерозом независимо от его локализации и/или степени выраженности, и специфические показатели, характеризующие либо локализацию, либо степень атеросклеротического поражения, либо и то, и другое.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To identify and characterize the associations of the presence and severity of atherosclerosis of various localization with the blood level of biochemical parameters, as well as to assess the potential of their use as markers of metabolic disorders with increased atherogenic potential.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 216 patients (men, 53%) aged 24-87 years (mean age, 61,5±10,73 years). All patients underwent coronary angiography, carotid (CA) and femoral arterial (FA) duplex ultrasound to assess the presence and severity of atherosclerosis. In blood serum/plasma, biochemical parameters were analyzed using standard methods.</p></sec><sec><title>Results</title><p>Results. Based on the analysis of circulating biomarker profile, diagnostic complexes have been established that allow assessing atherosclerosis of different localization. According to the data obtained, the determinants of coronary and CA atherosclerosis are endothelial dysfunction (concentration of nitric oxide metabolites &lt;36,0 μmol/L) and an increased level of creatinine (≥73,0 μmol/L). The specific markers associated with severe atherosclerosis of coronary and FAs (but not CA) were low high-density lipoprotein cholesterol (≤1,0/1,2 μmol/L for male/ female, respectively) and an increased C-reactive protein level (≥1,0 mg/l). Severe peripheral atherosclerosis (CA and FA involvement) was associated with hyperglycemia (glucose ≥6,1 μmol/L), while severe FA atherosclerosis — with hyperinsulinemia (insulin ≥14,0 μU/ml).</p></sec><sec><title>Conclusion</title><p>Conclusion. The analysis of associations of circulating biochemical parameters with atherosclerosis localization and severity revealed a number of metabolic markers associated with the increased atherogenic potential.
