Изучение гиполипидемического и плейотропных вазопротективных эффектов аторвастатина в зависимости от достижения целевого уровня (ЦУ) холестерина (ХС) липопротеинов низкой плотности (ЛНП) у больных инфарктом миокарда с подъемом сегмента ST в течение 48 нед. наблюдения.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Обследованы 125 больных инфарктом миокарда с подъемом сегмента ST, по результатам рандомизации получавших аторвастатин 40 или 80 мг/сут. 48 нед. На 7-9 сут., через 24, 48 нед. проводили биохимический анализ крови, эхокардиографию, исследование сонных артерий технологией высокочастотного сигнала и эндотелиальной функции. Обследуемых разделили на группы: высокоэффективной терапии (ВЭТ) — 41 пациент, достигший ЦУ ХС ЛНП на контрольных визитах; относительно эффективной (ОЭТ) — 35 больных, достигших ЦУ на одном визите; недостаточно эффективной (НЭТ) — 49 человек, не достигших ЦУ ХС ЛНП. При статистической обработке данных за достоверные принимали значения р&lt;0,05.</p></sec><sec><title>Результаты</title><p>Результаты. Обнаружено уменьшение частоты выявления повышенного уровня мозгового натрийуретического пептида в группах ВЭТ с 41,5 до 17% (р&lt;0,01), ОЭТ с 48,6 до 23% (р&lt;0,01), без динамики в группе НЭТ. Скорость клубочковой фильтрации в группе НЭТ снизилась на 8% (р&lt;0,01). В группе ВЭТ выявлено уменьшение артериального эластанса на 9,4%, толщины комплекса интима-медиа на 9,9%, снижение частоты отрицательной и прирост положительной реакции по индексу реактивности (р&lt;0,05).</p></sec><sec><title>Заключение</title><p>Заключение. Результаты демонстрируют важность достижения ЦУ ХС ЛНП для максимально благоприятной динамики мозгового натрийуретического пептида, структурно-функциональных характеристик артериальной системы.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the lipid-lowering and pleiotropic vasoprotective effects of atorvastatin depending on the achievement of the target low-density lipoprotein cholesterol (LDL-C) level in patients after ST-segment elevation myocardial infarction (STEMI) within 48-week follow-up period.</p></sec><sec><title>Material and methods</title><p>Material and methods. A total of 125 patients with STEMI, randomized to receive atorvastatin 40 or 80 mg per day for 48 weeks, were examined. On days 7-9, after 24, 48 weeks, we performed biochemical blood tests, echocardiography, as well as assessed the carotid arteries and endothelial function. The subjects were divided into the following groups: high-efficiency therapy (HET) — 41 patients who reached target LDL-C at control visits; moderate-efficiency therapy (MET) — 35 patients who achieved target LDL-C at one visit; low-efficiency therapy effective (LET) — 49 people who did not reach the target LDL-C. Differences were considered significant at p&lt;0,05.</p></sec><sec><title>Results</title><p>Results. A decrease in detection rate of an elevated brain natriuretic peptide was found in HET group from 41,5 to 17% (p&lt;0,01) and in MET group from 48,6 to 23% (p&lt;0,01), while no changes in the LET were revealed. The glomerular filtration rate in the LET group decreased by 8% (p&lt;0,01). In the HET group, a decrease in arterial elastance by 9,4%, intima-media thickness by 9,9%, a decrease in the frequency of a negative response and an increase in a positive response (p&lt;0,05) were revealed.</p></sec><sec><title>Conclusion</title><p>Conclusion.
