Установить факторы, ассоциированные с наличием дефицита железа (ДЖ) у больных с хронической сердечной недостаточностью (ХСН).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Обследовано 294 больных с ХСН II-IV функционального класса по NYHA (New York Heart Association), из которых 213 пациентов имели ДЖ, 81 — без ДЖ. Всем больным проводили клиническое обследование, сбор анамнеза с учетом наличия, длительности и тяжести коморбидных состояний, лекарственный анамнез, эхокардио,0графию, общий анализ крови, определение в сыворотке крови уровня железа, ферритина, трансферрина, насыщения трансферрина железом, растворимых рецепторов трансферрина, гепсидина, эритропоэтина, рутинные лабораторные исследования, определяли наличие и выраженность астении, тревоги, депрессии.</p></sec><sec><title>Результаты</title><p>Результаты. Установлено 29 клинических, лабораторных и эхо- кардиографических показателей, имеющих достоверные различия в группах больных с ДЖ и без ДЖ и достоверные корреляции с ДЖ. Определение отношения шансов для данных 29 параметров вы- явило 7 факторов, достоверно ассоциированных с наличием ДЖ у больных с ХСН. С помощью многофакторного логистического регрессионного анализа установлено достоверное совокупное влияние данных факторов на вероятность развития ДЖ.</p></sec><sec><title>Заключение</title><p>Заключение. ДЖ у больных с ХСН ассоциирован с возрастом, III-IV функциональным классом ХСН, повышенным уровнем гепсидина, длительностью анамнеза гипертонической болезни и сахарного диабета, анамнезом использования &gt;1 года ингибиторов ангиотензинпревращающего фермента и новых оральных антикоагулянтов. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To establish the factors associated with iron deficiency (ID) in pa- tients with heart failure (HF).</p></sec><sec><title>Material and methods</title><p>Material and methods. We examined 294 patients with NYHA class II-IV HF as follows: 213 — with ID, 81 — without ID. All patients underwent a clinical examination, collection of medical history taking into account the presence, duration and severity of comorbidities, drug history, echocardiography, complete blood count, serum iron, ferritin, transferrin, transferrin saturation, soluble transferrin receptors, hepcidin, erythropoietin, and routine laboratory tests. In addition, we determined the presence and severity of asthenia, anxiety, and depression.</p></sec><sec><title>Results</title><p>Results. Twenty-nine clinical, laboratory and echocardiographic parameters were established, which had significant differences in patients with and without ID and significant correlations with ID. Determining the odds ratio for these 29 parameters revealed 7 factors significantly associated with ID in patients with HF. Using multivariate logistic regression, a significant cumulative effect of these factors on the ID risk was established.</p></sec><sec><title>Conclusion</title><p>Conclusion.
