Изучить факторы сердечно­-сосудистого риска, клинические проявления и тактику ведения инфаркта миокарда (ИМ) у пациен­тов старческого возраста и долгожителей в зависимости от гериа­трического статуса.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Из 92 пациентов с ИМ, средний возраст 81,6±4,2, сформированы 3 группы: "Синдром старческой астении (ССА)" — 35 (38,0%), "Преастения" — 16 (17,4%) и "Без ССА" — 41 (44,6%).</p></sec><sec><title>Опросники</title><p>Опросники: "Возраст не помеха", краткая шкала оценки питания (Mini Nutritional Assessment, MNA), индекс Бартел, шкала Лоутона, тест рав­новесия, скорости ходьбы, "Встань и иди", динамометрия, краткая шка­ла оценки психического статуса (Mini Mental State Examination, MMSE), тест рисования часов, гериатрическая шкала депрессии.</p></sec><sec><title>Результаты</title><p>Результаты. Шанс выявления у пациента ССА выше при наличии ожирения, сахарного диабета (СД) 2 типа, снижении скорости клу­бочковой фильтрации (СКФ) &lt;60 мл/мин/1,73 м2, перенесенном инсульте, наличии сердечной недостаточности. При возникнове­нии клинических проявлений острого коронарного синдрома (ОКС) больные без ССА раньше обращались за помощью (в первые 2 ч — 26,8% пациентов (χ2=7,8, p=0,005). Большинство пациентов с пре­астенией и ССА вызывали медицинскую помощь позднее, но в пер­вые 12 ч — 68,8 и 74,3%, соответственно (χ2=15,6, p=0,012). Частота проведения тромболизиса — 23,2%, чрескожного коронарного вмешательства — 30,4% и не зависела от гериатрического статуса. Шансы обнаружить многососудистое поражение выше у пациентов с мальнутрицией или риском ее развития. Основные показатели гемодинамики, эхокардиографии, лабораторные показатели у па­циентов с ИМ не зависели от гериатрического статуса. Согласно оценке STOPP/START — критериев больные с ССА чаще получали в стационаре нерекомендованные лекарства.</p></sec><sec><title>Заключение</title><p>Заключение. ССА у пациентов с ИМ был ассоциирован с множе­ственными факторами сердечно-­сосудистого риска, более позд­ним обращением за медицинской помощью и более частым ис­пользованием нерекомендованных лекарственных средств.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study cardiovascular risk factors, clinical manifestations and management of myocardial infarction (MI) in elderly and long­living patients depending on geriatric status.</p></sec><sec><title>Material and methods</title><p>Material and methods. Out of 92 patients with MI (mean age, 81,6±4,2), 3 following groups were formed: "frailty" — 35 (38,0%), "prefrailty" — 16 (17,4%), and "no frailty" — 41 (44,6%). The following questionnaires were used: Age Is Not a Hindrance, Mini Nutritional Assessment (MNA), Barthel Index for Activities of Daily Living, Lawton Instrumental Activities of Daily Living Scale, balance test, Gait Speed Test, Timed Up and Go test, grip test, Mini Mental State Examination (MMSE), Clock Drawing Test, Geriatric Depression Scale.</p></sec><sec><title>Results</title><p>Results. Frailty probability in patients is higher in the presence of obesity, type 2 diabetes, a decrease in glomerular filtration rate (GFR) &lt;60 ml/min/1,73 m2, a stroke, and heart failure. In the event of clinical manifestations of acute coronary syndrome (ACS), patients without frailty sought medical assistance earlier (in the first 2 hours — 26,8% of patients (χ2=7,8, p=0,005). Most patients with prefrailty and frailty called an ambulance later, but in the first 12 hours — 68,8 and 74,3%, respectively (χ2=15,6, p=0,012). The prevalence of thrombolysis is 23,2%, percutaneous coronary intervention — 30,4% and did not depend on geriatric status. The probability of multivessel disease is higher in patients with or at risk of malnutrition. The main hemodynamic parameters, echocardiography, laboratory parameters in patients with MI did not depend on geriatric status. According to the STOPP/START criteria, frail patients with more often received nonrecommended inhospital medicines.</p></sec><sec><title>Conclusion</title><p>Conclusion.
