Оценить степень достижения целевых значений лабораторных показателей на фоне антикоагулянтной терапии у пациентов с тромбозом глубоких вен нижних конечностей (ТГВНК).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Работа выполнена в дизайне одномоментного, описательного, аналитического, фармакоэпидемиологического исследования. Материалом для исследования послужили 200 историй болезни пациентов с ТГВНК, находящихся на лечении в стационарах г. Волгограда. Проанализирована структура назначения антикоагулянтов, выполнен анализ лабораторных показателей коагулограммы: активированного частичного тромбопластинового времени (АЧТВ), протромбинового индекса и международного нормализованного отношения (МНО).</p></sec><sec><title>Результаты</title><p>Результаты. Антикоагулянты прямого действия были назначены в 91% случаев. На долю нефракционированного гепарина (НФГ) пришлось 84,5% назначений, на долю низкомолекулярных гепаринов — 6,5%. Мониторинг лабораторного теста АЧТВ осуществлялся только у 36% пациентов. Рекомендуемое увеличение показателя АЧТВ в 1,5–2,5 раза от верхней границы нормы достигнуто лишь в 6% случаев. Из антикоагулянтов непрямого действия был назначен в 75,5% случаев варфарин. Несмотря на то, что лабораторный контроль лечения варфарином осуществлялся в 97% случаев, лишь у 28% пациентов показатели МНО перед выпиской находились в рекомендованном “терапевтическом” коридоре 2,0–3,0.</p></sec><sec><title>Заключение</title><p>Заключение. Полученные результаты свидетельствуют о том, что антикоагулянтная терапия при ТГВНК имеет ряд недостатков. Назначение низких доз НФГ и варфарина, недостаточно активный мониторинг гипокоагуляции, отсутствие коррекции фармакотерапии (титрование дозы) в зависимости от полученных лабораторных результатов привели к тому, что оптимальный “терапевтический” диапазон гипокоагуляции, который обеспечивает максимальную эффективность и безопасность антикоагулянтной терапии, не был достигнут. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To evaluate the prevalence of accomplished goal in laboratory clotting parameters for the anticoagulant therapy in deep vein thrombosis (DVT).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study is performed as one-stage, descriptive, analytic, pharmacoepidemiologic investigation. The material for the study were 200 case histories of DVT from Volgograd hospitals. The analyzed were: structure of anticoagulant prescription, coagulogrammes with APTT, prothrombin complex and INR.</p></sec><sec><title>Results</title><p>Results. Direct acting anticoagulants were prescribed in 91% cases. High-molecular heparin was used in 84,5% prescriptions, low-molecular heparins — in 6,5%. Monitoring of APTT was done only in 36% patients. The target values of APTT (1,5–2,5 times higher than upper limit) was reached only in 6% cases. Of the indirect anticoagulants warfarin was used in 75,5%. Although laboratory control of warfarin was done in 97% cases, only in 28% of patients INR at discharge was in the range 2,0–3,0.</p></sec><sec><title>Conclusion</title><p>Conclusion. The data witnesses a variety of drawbacks in anticoagulant therapy of DVT patients.
