Определить, соответствует ли уровень сердечно-сосудистого риска, рассчитанный по шкале SCORE, оценке уровня коронарного кальция, рассчитанного по методике Агатстона.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включено 212 человек в возрасте 40-65 лет, средний возраст составил 56,5±7,9 лет. Количество мужчин составило 54 (25,5%) человека, а женщин 158 (74,5%).</p></sec><sec><title>Результаты</title><p>Результаты. По шкале SCORE (Systematic Coronary Risk Evaluation) группы распределились следующим образом: 62 (29,2%) человека имели низкий риск, 128 (60,4%) человек — умеренный риск, 16 (7,5%) человек имели высокий риск и 6 (2,8%) — очень высокий риск. Средняя оценка по SCORE во всей группе составила 2,5±2,4%. По уровню кальциевого индекса (КИ) или индекса Агатстона, группы распределились следующим образом: минимальный риск 0-10 единиц (ед.) — 142 (67%) человека, низкий риск 11-100 ед. — 42 (19,8%) человека, умеренный риск 101-400 ед. — 17 (8%) человек и высокий риск ≥401 ед. — 7 (3,3%) человек. При сравнении групп, сформированных по уровню риска, рассчитанному по шкале SCORE и показателям КИ, выявлено несоответствие данных по всем категориям сердечно-сосудистого риска.</p></sec><sec><title>Заключение</title><p>Заключение. Выявленные несовпадения распределения по группам риска в соответствии со шкалой SCORE и по КИ свидетельствуют о недостаточной информативности шкалы SCORE. При определении у пациентов уровня риска рекомендовано дополнительное проведение мультиспиральной компьютерной томографии коронарных сосудов с расчетом КИ, что позволит определить тактику ведения пациента и решить вопрос о назначении медикаментозного лечения. Сравнительный анализ диагностической ценности КИ и шкалы SCORE может помочь оптимизировать, прежде всего, медикаментозную терапию пациентов с артериальной гипертонией и нарушениями липидного обмена.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To determine whether the Systematic Coronary Risk Evaluation (SCORE) level corresponds to the coronary artery calcium (CAC) score (Agatston score).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 212 people aged 40-65 years (mean age, 56,5±7,9 years). The number of men and women was 54 (25,5%) and 158 (74,5%), respectively.</p></sec><sec><title>Results</title><p>Results. According to the SCORE, the groups were distributed as follows: 62 (29,2%) — low risk, 128 (60,4%) — moderate risk, 16 (7,5%) — high risk, 6 (2,8%) — very high risk. The average SCORE level for the general group was 2,5±2,4%. According to the Agatston score, the groups were distributed as follows: minimal risk (0-10) — 142 (67%) people, low risk (11-100) — 42 (19,8%) people, moderate risk (101-400) — 17 (8%) people, high risk (≥401) — 7 (3,3%) people. Inconsistencies for all categories of cardiovascular risk were revealed between SCORE and Agatston score.</p></sec><sec><title>Conclusion</title><p>Conclusion. The identified inconsistencies in the distribution of risk groups in accordance with the SCORE and Agatston score indicate that the SCORE scale is insufficiently informative. Multislice computed tomography coronary angiography with CAC calculation is additionally recommended, which will allow determining patient management and deciding on therapy.
