Определение взаимосвязей уровня нейрегулина-1 (NRG-1) в сыворотке крови с выраженностью атеросклеротического поражения коронарных артерий (КА) и клинико-инструментальными характеристиками пациентов с ишемической болезнью сердца (ИБС).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 264 человека, из них 220 пациентов с диагнозом ИБС. Пациентам была выполнена коронароангиография с использованием шкалы SYNTAX (Synergy between Percutaneous Coronary Intervention with TAXUS and Cardiac Surgery) и эхокардиографическое исследование. Больные были разделены на группы согласно баллам по шкале SYNTAX: 1 группа — с умеренным атеросклеротическим поражением КА (≤ 22 баллов — 124 человека); 2 группа — с выраженным атеросклерозом КА (23-32 балла — 53 человека); 3 группа — с крайне тяжелым поражением КА (≥33 баллов — 43 человека). Группа 4 была представлена здоровыми добровольцами (44 человека). У всех испытуемых проведено исследование уровня NRG-1 (нг/мл) в сыворотке крови. Статистическую обработку результатов осуществляли с использованием программного обеспечения "Statistica 10.0"; достоверными считали различия при р&lt;0,05.</p></sec><sec><title>Результаты</title><p>Результаты. Получены статистически значимо более высокие показатели NRG-1 в группе контроля в сравнении с пациентами с ИБС (p&lt;0,001). Обнаружена обратная корреляционная связь между концентрацией NRG-1 и выраженностью поражения КА (p&lt;0,001). Выявлено, что уменьшение концентрации NRG-1 ассоциируется с высоким функциональным классом хронической сердечной недостаточности (p&lt;0,01) и низкой фракцией выброса левого желудочка (p&lt;0,001). Необходимо отметить достоверность различий показателей NRG-1 между группами пациентов с наличием инфаркта миокарда в анамнезе (p&lt;0,001), стенокардией (p&lt;0,01), постоянной формой фибрилляции предсердий (p&lt;0,01), хронической аневризмой левого желудочка (p&lt;0,01) и повторными инфарктами миокарда (p&lt;0,05) в сравнении с пациентами без данных патологий.</p></sec><sec><title>Заключение</title><p>Заключение. Выявленные корреляционные связи между концентрацией NRG-1 и поражением КА, а также клинико-инструментальными характеристиками пациентов позволяют рассматривать NRG-1 в качестве надежного биомаркера выраженности коронарного атеросклероза и тяжести хронической сердечной недостаточности и могут явиться основой для разработки новых диагностических подходов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To determine the relationship between the serum level of neu-regulin-1 (NRG-1) and the severity of coronary artery (CA) atherosclerosis and clinical and paraclinical characteristics of patients with coronary artery disease (CAD).</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 264 people, of which 220 were patients diagnosed with coronary CAD. The patients underwent coronary angiography using the Synergy between Percutaneous Coronary Intervention with TAXUS and Cardiac Surgery (SYNTAX) score and echocardiography. The patients were divided into groups according to SYNTAX score: group 1 — with moderate CA atherosclerosis (≤22 (n=124); group 2 — with severe CA atherosclerosis (23-32) (n=53); group 3 — with extremely severe CA atherosclerosis (≥33) (n=43). Group 4 was represented by healthy volunteers (n=44). All subjects underwent a study of the serum NRG-1 (ng/ml) level. Statistical processing of the results was carried out using Statistica 10.0 software. Differences were considered significant at p&lt;0,05.</p></sec><sec><title>Results</title><p>Results. Significantly higher NRG-1 values were obtained in the control group compared to patients with CAD (p&lt;0,001). An inverse correlation was found between the NRG-1 concentration and the severity of CA atherosclerosis (p&lt;0,001). A decrease in NRG-1 concentration is associated with a high functional class of = heart failure (p&lt;0,01) and low left ventricular ejection fraction (p&lt;0,001). The significance of NRG-1 differences between groups of patients with a history of myocardial infarction (p&lt;0,001), angina pectoris (p&lt;0,01), permanent atrial fibrillation (p&lt;0,01), chronic left ventricular aneurysm (p&lt;0,01) and repeated myocardial infarction (p&lt;0,05) in comparison with patients without these pathologies.</p></sec><sec><title>Conclusion</title><p>Conclusion.
