Изучение взаимосвязи интегральных метаболических индексов с величиной CAVI (Cardio-Ankle Vascular Index) и оценка их предиктивной ценности для выявления повышенного уровня артериальной жесткости у лиц молодого и среднего возраста с артериальной гипертонией и другими факторами риска сердечно-сосудистых заболеваний.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование было включено 160 паци­ентов (56 мужчин и 104 женщины), медиана возраста составила 39 [22,0;57,0] лет. Всем пациентам проведено антропометрическое обследование, исследование биохимических показателей липидного профиля — уровня общего холестерина (ХС), ХС липопротеинов низкой и высокой плотности и триглицеридов, с помощью экспресс-анализатора CardioChek PA (США, 2020г) с последующим расчетом индексов LAP (Lipid Accumulation Product), VAI (Visceral Adiposity Index), BFP (Body Fat Percentage), BAI (Body Adiposity Index). Наличие повышенной артериальной жесткости оценивалось по уровню CAVI методом сфигмоманометрии на аппарате VaSera-1000 FUCUDA DENSHI (Япония, 2010г). Статистический ана­лиз результатов проводился с помощью программы Jamovi.</p></sec><sec><title>Результаты</title><p>Результаты. Частота абдоминального ожирения, артериальной гипертензии, дислипидемии и курения соответствовала общепопуляционной. Достоверных взаимосвязей CAVI с антропометрическими метаболическими маркерами — весом, индексом массы тела, окружностью талии, окружностью бедер, окружностью шеи, получено не было (p&gt;0,05). Выявлены статистически значимые прямые корреляционные взаимосвязи CAVI c уровнями общего ХС, ХС липопротеинов низкой плотности и интегральными метаболическими индексами (LAP, VAI и BFP). Наиболее сильная корреляционная взаимосвязь была получена между CAVI и LAP, BFP. По результатам многофакторного анализа из оцениваемых инте­гральных метаболических индексов независимая взаимосвязь с CAVI выявлена только у LAP и VAI, для которых был проведен ROC-анализ. При увеличении LAP-индекса &gt;39 с чувствительностью 75,24% и специфичностью 77,68% (AUC, (Area Under the Curve) 0,859, пороговое значение =39) можно выявить повышенную артериальную жесткость. Для VAI была получена модель с высокой специфичностью 100%, но низкой чувствительностью 18,2% (AUC 0,617), что указывает на нецелесообразность использования этого маркера.</p></sec><sec><title>Заключение</title><p>Заключение. Для выявления раннего сосудистого старения у лиц молодого и среднего возраста помимо традиционных метаболических маркеров может использоваться интегральный метаболический индекс LAP, который при его значении &gt;39 позволяет с чувствительностью 75,24% и специфичностью 77,68% предсказать наличие высокой артериальной жесткости.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the relationship of integral metabolic indices with the Cardio-Ankle Vascular Index (CAVI) and assess their predictive value for identifying increased levels of arterial stiffness in young and middle-aged people with hypertension and other cardiovascular risk factors.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study included 160 patients (56 men and 104 women) with the median age of 39 [22,0;57,0] years. All patients underwent an anthropometric measurement and a study of lipid profile (total cholesterol (TC), low- and high-density lipoprotein cholesterol and triglycerides) using the CardioChek PA analyzer (USA, 2020) with subsequent calculation of lipid accumulation product (LAP), visceral adiposity index (VAI), body fat percentage (BFP), body adiposity index (BAI). Increased arterial stiffness was assessed by the CAVI level by the sphygmomanometry using the VaSera-1000 FUCUDA DENSHI system (Japan, 2010). Statistical analysis was carried out using the Jamovi program.</p></sec><sec><title>Results</title><p>Results. The incidence of abdominal obesity, hypertension, dyslipidemia and smoking was consistent with the general population. There were no significant relationships between CAVI and anthropometric metabolic markers (weight, body mass index, waist circumference, hip circumference, neck circumference) (p&gt;0,05). Significant direct correlations between CAVI and the levels of TC, low-density lipoprotein cholesterol and integral metabolic indices (LAP, VAI and BFP) were revealed. The strongest correlation was obtained between CAVI and LAP, BFP. According to multivariate analysis, an independent relationship with CAVI was detected only in LAP and VAI, for which ROC analysis was performed. With LAP &gt;39, increased arterial stiffness can be detected with a sensitivity of 75,24% and specificity of 77,68% (Area Under the Curve (AUC) 0,859, threshold =39). For VAI, a model with a high specificity of 100% but a low sensitivity of 18,2% (AUC 0,617) was obtained.</p></sec><sec><title>Conclusion</title><p>Conclusion.
