Изучить долгосрочное влияние комплексной терапии с добавлением усиленной наружной контрпульсации (УНКП) на клинические показатели и исходы у пациентов со стабильной ишемической болезнью сердца (ИБС), осложненной хронической сердечной недостаточностью (ХСН).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В рандомизированном исследовании EXCEL (Long-term Effects of enhanced eXternal CountErpuLsation; NCT05913778) 118 пациентов со стабильной ИБС, осложненной ХСН II-III функционального класса по NYHA (New-York Heart Association), рандомизированы в 1-ю группу (n=59) — оптимальная медикаментозная терапия и УНКП (35 ч, 2 курса/год) или 2-ю группу (n=59) — оптимальная медикаментозная терапия и УНКП (35 ч, 1 курс/год). Ежегодно проводили тест с 6-минутной ходьбой (6МХТ), оценку клинического статуса, качества жизни (опросник MLHFQ), уровня NT-proBNP (N-концевой промозговой натрийуретический пептид), фракции выброса левого желудочка и регистрацию клинических исходов.</p></sec><sec><title>Результаты</title><p>Результаты. В обеих группах отмечена положительная динамика ФК ХСН, клинического статуса, толерантности к нагрузке, уровня NT-proBNP, качества жизни. Первичная конечная точка (КТ) — увеличение дистанции в 6МХТ &gt;20% — в 1-й и 2-й группах зарегистрирована у 100 и 79,7% пациентов, соответственно (p&lt;0,001), комбинированная КТ (инфаркт миокарда, реваскуляризация, смерть от всех причин, госпитализации по поводу ХСН, новые случаи фибрилляции предсердий, сахарного диабета 2 типа, хронической болезни почек 3-5 стадий) у 18,6 и 52,5%, соответственно. Отношение шансов развития первичной КТ в 1-й группе составило 0,207 (95% доверительный интервал: 0,090-0,475; p&lt;0,001), комбинированной КТ — 0,032 (95% доверительный интервал 0,002-0,553; p=0,018).</p></sec><sec><title>Заключение</title><p>Заключение. В исследовании EXCEL продемонстрировано стойкое положительное влияние УНКП у пациентов с ХСН, осложнившей ИБС, на клинические показатели и исходы, которые были значимо более выражены в группе с 2 курсами УНКП (70 ч) в год по сравнению с однократным ежегодным курсом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the long-term effect of complex therapy with enhanced external counterpulsation (EECP) on clinical parameters and outcomes in patients with stable coronary artery disease (CAD) complicated by heart failure (HF).</p></sec><sec><title>Material and methods</title><p>Material and methods. The randomized trial EXCEL (Long-term Effects of enhanced eXternal CountErpuLsation; NCT05913778) included 118 patients with stable CAD complicated by New-York Heart Association (NYHA) class II-III HF, who were randomized into group 1 (n=59) receiving optimal therapy and EECP (35 hours, 2 courses/ year) or group 2 (n=59) receiving optimal therapy and EECP (35 hours, 1 course/year). The 6-minute walk test (6MWT), clinical status, quality of life (MLHFQ questionnaire), N-terminal pro-brain natriuretic peptide (NT-proBNP) level, left ventricular ejection fraction, and clinical outcomes were assessed annually.</p></sec><sec><title>Results</title><p>Results. In both groups, improvement of HF functional class, clinical status, exercise tolerance, NT-proBNP level, and quality of life were noted. Primary endpoint (increase in 6MWT distance &gt;20%) in groups 1 and 2 was recorded in 100 and 79,7% of patients, respectively (p&lt;0,001), while composite endpoint (myocardial infarction, revascularization, all-cause death, hospitalization for HF, new cases of atrial fibrillation, type 2 diabetes, stage 3-5 chronic kidney disease) in 18,6 and 52,5%, respectively. The odds ratio for primary and composite endpoint in group 1 was 0,207 (95% confidence interval: 0,090-0,475; p&lt;0,001) and 0,032 (95% confidence interval 0,002-0,553; p=0,018).</p></sec><sec><title>Conclusion</title><p>Conclusion.
