Проанализировать особенности и динамику ранних нарушений углеводного обмена и инсулинорезистентности (ИР) при разном уровне приверженности лечению (ПЛ) у лиц молодого возраста.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Обследовано 94 пациента (45 мужчин/49 женщин) в возрасте — медиана (Ме)=33 [28,7-38] лет. Дизайн — проспективное наблюдательное исследование с изучением ПЛ пациентов по КОП-25 (опроснику количественной оценки приверженности к лечению), уровню глюкозы плазмы натощак, результатам перорального глюкозотолерантного теста, уровням гликированного гемоглобина (HbA1с), инсулина и HOMA (Homeostasis Model Assessment of Insulin Resistance) индекса. Статистическая обработка проведена в IBM SPSS Statistics 26.</p></sec><sec><title>Результаты</title><p>Результаты. Только 15,8% имели высокую приверженность медицинскому сопровождению, 8,9% — модификации образа жизни (ПМ) и общую ПЛ, 10,9% — лекарственной терапии. Средние значения в большинстве типов приверженности находились в диапазоне низких величин. В группах низкой и средней ПМ за 18 мес. увеличилось количество пациентов с повышенным уровнем HbA1с с 7,1 до 18,8% (р=0,021) и ИР с 12% до 31,8% (р=0,000). У пациентов с низкой общей ПЛ в сравнении с высокой возрастал шанс повышения уровня HbA1с в 5,5 раза, гиперинсулинемии — в 1,16 раза.</p></sec><sec><title>Заключение</title><p>Заключение. При 18-мес. проспективном исследовании у лиц молодого возраста с низким уровнем ПЛ наблюдается нарастание частоты гиперинсулинемии, ИР и предиабета.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To analyze early disorders of carbohydrate metabolism and insulin resistance (IR) at different compliance levels in young people.</p></sec><sec><title>Material and methods</title><p>Material and methods. Ninety-four patients (45 men/49 women; median (Me)=33 [28,7-38] years) were examined. This prospective observational study used data of the questionnaire of the quantitative assessment of the adherence to treatment (QAA-25), fasting plas­ma glucose levels, oral glucose tolerance test, levels of glycated hemo­globin (HbA1c), insulin and Homeostasis Model Assessment of Insulin Resistance (HOMA-IR). Statistical processing was carried out in IBM SPSS Statistics 26.</p></sec><sec><title>Results</title><p>Results. Only 15,8% had high adherence to medical support, 8,9% — lifestyle modification (LM) and general compliance, 10,9% — drug therapy. The means for most types of adherences were in the low range. In the low and medium LM groups, over 18 months the number of patients with elevated HbA1c levels increased from 7,1 to 18,8% (p=0,021) and IR from 12% to 31,8% (p=0,000). In patients with low overall LM compared to high ones, the probability of HbA1c increase raised by 5,5 times, and hyperinsulinemia by 1,16 times.</p></sec><sec><title>Conclusion</title><p>Conclusion.
