Данный патологический процесс приводит к смерти 10% реципиентов донорского сердца в течение 15 лет. Ранняя диагностика БКАПС способствует улучшению прогноза пациентов с трансплантированным сердцем. Коронароангиография имеет ограничения в оценке гиперплазии интимы у пациентов с БКАПС, в связи с чем методы внутрисосудистой визуализации и оценки коронарной физиологии являются незаменимыми помощниками в раннем выявлении этого состояния. Представлен клинический случай использования морфофункционального подхода к диагностике БКАПС. Для определения функциональной значимости стеноза было проведено инвазивное измерение фракционного резерва кровотока. Метод оптической когерентной томографии позволил оценить морфологический субстрат поражения и подтвердить аутоиммунный механизм его возникновения. Использование морфофункционального подхода к данной группе пациентов позволяет с большей точностью прогнозировать течение заболевания, а также при необходимости, проводить адекватное лечение.</p></abstract><trans-abstract xml:lang="en"><p>Cardiac allograft vasculopathy (CAV) remains an unsolved problem in modern transplantology. This pathological process leads to the death of 10% of heart recipients within 15 years. Early diagnosis of CAV improves the prognosis of transplanted heart patients. Coronary angiography has limitations in the assessment of intimal hyperplasia in CAV patients whereas intravascular imaging and coronary physiology assessment are essential aids in its early detection. The case of the using morphofunctional approach in CAV diagnosis is presented. To determine the functional significance of stenosis, fractional flow reserve was measured invasively. Optical coherence tomography allowed us to assess morphological substrate and confirm the autoimmune pathogenesis.
