Ассоциированные с АГ заболеваемость и смертность продолжают расти, несмотря на широкий спектр доступных антигипертензивных препаратов и регулярное обновление клинических рекомендаций по лечению и диагностике этого заболевания. В октябре 2024 года состоялся ежегодный форум спикеров, посвященный обсуждению оптимизации подходов к лечению больных АГ. В ходе дискуссии особое внимание было уделено персонифицированной терапии пациентов, которая должна быть основана на современных представлениях о патогенезе заболевания и индивидуальных факторах риска. Кроме того, обсуждалась обоснованность назначения в определенных клинических ситуациях, наряду с основными пятью классами антигипертензивных препаратов, агонистов имидазолиновых рецепторов, относящихся к дополнительным средствам, снижающим артериальное давление. В рутинной практике для контроля АГ в составе комбинированной терапии рекомендуется рассмотреть применение препаратов центрального действия, в частности моксонидина (препарат Физиотенз®). Высокий уровень блокады симпатической нервной системы при ее гиперактивности на фоне терапии моксонидином обеспечивает контроль уровня АД и обладает плейотропными эффектами, среди которых наибольшее значение имеет повышение чувствительности тканей к инсулину, что особенно важно для лечения АГ у пациентов с ожирением, метаболическим синдромом, предиабетом, сахарным диабетом 2 типа, у пациентов с различными метаболическими нарушениями (гипергликемией, дислипидемией, гиперурикемией) и у женщин в пери- и постменопаузальном периоде.</p></abstract><trans-abstract xml:lang="en"><p>Hypertension (HTN) remains one of the most pressing problems of modern healthcare. Morbidity and mortality associated with HTN continue to increase, despite a wide range of available antihypertensive agents and regular updates of related clinical guidelines. In October 2024, an annual forum was held to discuss the optimization of approaches to the treatment of HTN. During the discussion, special attention was paid to personalized therapy of patients, which should be based on modern concepts of the disease pathogenesis and individual risk factors. In addition, the validity of prescribing imidazoline receptor agonists, which are additional agents that lower blood pressure, in certain clinical situations was discussed, along with the main five classes of antihypertensive drugs. In routine practice, it is recommended to consider the use of centrally acting drugs, in particular moxonidine, to control hypertension as part of combination therapy. A central sympathetic inhibition with moxonidine therapy ensures control of blood pressure and has pleiotropic effects, among which the most important is an increase in tissue sensitivity to insulin.
