Изучить клинические результаты и частоту окклюзии шунтов у больных с диффузным поражением и малым (&lt;1,5 мм) диаметром шунтируемых коронарных артерий (КА) через год после коронарного шунтирования (КШ) при приеме различных вариантов антитромботической терапии (АТТ) — аспирина и комбинация аспирина с клопидогрелом).</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В проспективное рандомизированное исследование включены 94 пациента со стабильным течением ишемической болезни сердца и многососудистой коронарной болезнью, имеющие диффузное поражение и малый (&lt;1,5 мм) диаметр принимающего русла, которые перенесли неосложненное КШ. Рандомизация в 2 группы: группу АСК — монотерапия аспирином (ацетилсалициловой кислотой) (n=41) и группу АСК+Клоп — терапия аспирином в комбинации с клопидогрелом (n=53). Медиана наблюдения (интерквартильный размах) — 20 (12; 24 мес.); всем выполнена мультиспиральная компьютерная томография (МСКТ)шунтография.</p></sec><sec><title>Результаты</title><p>Результаты. Общее количество шунтов составило 349, суммарная частота их окклюзий — 54 (16%). Частота окклюзий шунтов в группах АСК и АСК+Клоп оказалась очень близкой: 19 vs 13% (р=0,191). При субанализе функции шунтов к артериям диаметром &lt;1,5 мм окклюзии развивались чаще в группе АСК: 39 vs 12% (р=0,005); такое различие сформировалось за счет "разрыва" между группами по частоте окклюзии шунтов к артериям наименьшего калибра (≲1 мм): 60 vs 17% (р=0,006). Частота рецидива стенокардии при монотерапии аспирином была несколько выше, чем при "усиленной" АТТ: 20 vs 13%, р=0,480.</p></sec><sec><title>Заключение</title><p>Заключение. Окклюзии шунтов к артериям малого (&lt;1,5 мм) диаметра, оцененные через год после КШ, развиваются достоверно реже на фоне терапии комбинацией АСК и клопидогрела.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the clinical outcomes and the frequency of bypass graft occlusions in patients with diffuse coronary artery disease (CAD) and small (&lt;1,5 mm) diameter of bypassed coronary arteries (CA) one year after coronary artery bypass grafting (CABG) while taking various antithrombotic therapies (ATT) (aspirin and a combination of aspirin with clopidogrel).</p></sec><sec><title>Material and methods</title><p>Material and methods. This prospective randomized study included 94 patients with a stable diffuse multivessel CAD with a small (&lt;1,5 mm) vessel diameter, who underwent uncomplicated CABG. The patients were randomized into 2 following groups: acetylsalicylic acid (ASA) monotherapy (n=41) and ASA+clopidogrel therapy (n=53). The median follow-up (interquartile range) was 20 (12; 24 months). All participants underwent computed tomographic (CT) bypass angiography.</p></sec><sec><title>Results</title><p>Results. There were a total of 349 bypasses with a total occlusion rate of 54 (16%). The bypass occlusion rate in the ASA and ASA+clopidogrel groups was very close as follows: 19 vs 13% (p=0,191). In the subanalysis of the bypass graft function related to arteries with a diameter &lt;1,5 mm, occlusions developed more often in the ASA group (39 vs 12%, p=0,005). This difference was formed due to the gap between the groups in the bypass graft occlusion rate related arteries of the smallest diameter (≲1 mm) (60 vs 17%, p=0,006). The angina recurrence rate with aspirin monotherapy was slightly higher than with "enhanced" ATT (20 vs 13%, p=0,480).</p></sec><sec><title>Conclusion</title><p>Conclusion.
